Quantum Restoration

This waiver applies to every private one on one session at Quantum Temple, located in Brickell, Miami, Florida, and to any private session held offsite. One signature covers all current and future private sessions.
Sessions covered include: Private Sound Healing, Quantum Restoration, Reiki, Family Constellations, Spiritual Response Therapy, Quantum Reading, Couple Sessions, and the Blossom Experience.
Please read each section carefully and initial where indicated before your first session.
Nature of the Services
Quantum Temple offers holistic and spiritual wellness practices intended to support relaxation, self awareness, and personal growth.
I understand that:
• These sessions are not medical, psychological, or psychiatric treatment, and are not a substitute for care from a licensed physician or mental health professional.
• My practitioner does not diagnose conditions, prescribe medication, or promise any specific outcome.
• I will continue any treatment prescribed by my healthcare providers and will not stop medication or care because of a session.
• Practices such as Reiki, Family Constellations, Spiritual Response Therapy, and Quantum Reading are complementary and spiritual in nature, and I participate by my own choice.

Health Disclosure
Sessions may use vibroacoustic water beds, gongs, crystal bowls, and other instruments that create sound and physical vibration. I agree to tell my practitioner before the session if any of the following apply to me, and to consult my doctor before participating:

[ ] Blood clots or deep vein thrombosis
[ ] Recent surgery or injury

[ ] A mental health condition for which I am currently under care
I confirm the information I share is accurate, and I will update Quantum Temple if my health changes. I may stop or adjust any session at any time if I feel uncomfortable.

Assumption of Risk
I understand that sessions may bring up physical sensations such as tingling, warmth, dizziness, or temporary discomfort, as well as strong emotions, memories, or deep release. These responses are a natural part of the process.
I understand that sessions involving water beds, floating, mats, or lying down carry minor risks such as slipping or discomfort when getting up. I will move slowly, follow my practitioner's guidance, and let them know if I need assistance.
I voluntarily accept these risks and take full responsibility for my wellbeing during and after each session.

Release of Liability
To the fullest extent permitted by Florida law, I release and hold harmless Quantum Temple, its owner, practitioners, guest facilitators, staff, and partner venues from any claims, injuries, losses, or damages arising from my participation in private sessions, except those caused by gross negligence or intentional misconduct.
This release applies to me, my heirs, and anyone acting on my behalf. If any part of this waiver is found unenforceable, the remaining parts stay in full effect.

Session Policies
Cancellations. Please give at least [24/48] hours notice to cancel or reschedule. Late cancellations and no shows are charged [full / 50% of] the session price.
Payment. Payment is due [at booking / before the session begins]. Packages and the Blossom Experience are nonrefundable once started.
Confidentiality. Everything shared during a session is kept private, unless disclosure is required by law or there is a risk of serious harm to me or others.
Physical touch. Some sessions, such as Reiki, may include light, nonintrusive touch. I may ask for a hands off session at any time.
Photos and video. Quantum Temple will never record a private session without my consent.
