This Agreement is between The Home Stretch Chicago LLC ("Service Provider") and the client.

1. Acknowledgment of Risks
I acknowledge that participating in assisted stretching sessions involves physical activity that carries inherent risks, including muscle strains, joint injuries, and emotional stress. I understand that The Home Stretch Chicago LLC will take reasonable precautions, but I assume full responsibility for any injury or discomfort that may occur.

2. Health Declaration
I affirm that I am in good health and capable of participating in assisted stretching. I have disclosed any relevant medical conditions or injuries, and will notify the Service Provider if anything changes.

3. Waiver of Liability
In consideration of participating in these services, I release and hold harmless The Home Stretch Chicago LLC, its employees, agents, and affiliates from any liability for injuries, damages, or losses that may occur during or after the session, including physical and emotional injuries.

4. Indemnification
I agree to indemnify and hold harmless The Home Stretch Chicago LLC from any claims, damages, or expenses arising out of my participation in the stretching sessions.

5. Voluntary Participation
I am participating voluntarily and acknowledge that by signing this document, I am waiving certain legal rights, including the right to sue.


50 Minute Stretch Session

In-home sessions are available in River North and Gold Coast. Wear comfortable clothing and socks—grippy socks are great but not necessary. As long as there’s a bit of space for the stretching table, I’ll handle the rest!
