By signing this waiver, I acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in an AquaBoard Pilates class involves physical activity performed on water-based equipment in and around a swimming pool, which carries inherent risks.

These risks include, but are not limited to:
	•	Slipping or falling on wet surfaces
	•	Loss of balance while exercising on the AquaBoard
	•	Muscle strains, sprains, or other physical injuries
	•	Fatigue, dehydration, or overexertion
	•	Risks associated with being in or around water

I voluntarily assume all risks associated with participating in this activity, whether known or unknown.

2. Health Acknowledgment

I certify that I am physically fit and in good health to participate in this class.

I understand that it is my responsibility to stop exercising if I feel pain, dizziness, discomfort, or any abnormal symptoms.

I acknowledge that Glee Concept LLC, Genesis Health Clubs, and the instructor are not responsible for monitoring my physical condition during the class.

3. Release of Liability

I hereby release, waive, and discharge Glee Concept LLC, Genesis Health Clubs, the class instructor, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or connected with my participation in the AquaBoard Pilates class.

This includes any injuries, damages, or losses that may occur during the class to the fullest extent permitted by law.

4. Pool Area Safety

I understand that the class takes place in and around a pool area, where surfaces may be wet and slippery.

I agree to follow all safety instructions provided by the instructor and facility staff.

I understand that Glee Concept LLC and Genesis Health Clubs are not responsible for lost, stolen, or damaged personal belongings during the class.

6. Photo & Video Release

I grant permission for Glee Concept LLC and its partners to take photographs or videos during the class and use them for marketing, promotional, and social media purposes without compensation.

7. Legal Agreement

I agree that this waiver shall be governed by the laws of the State of Florida.

Any disputes arising from participation in this activity shall be resolved in Orange County, Florida.
