Build Upper is an upper-body strength and muscle development class focused on building power, definition, and structural balance through intentional resistance training. This session targets the chest, back, shoulders, arms, and core using a combination of compound lifts, hypertrophy-focused movements, and functional accessory work to improve strength, posture, and overall upper-body performance. Participants can expect a challenging yet scalable workout designed to increase muscular endurance, enhance stability, and develop a strong, athletic physique. Build Upper is centered on controlled intensity, quality movement, and consistent progression.

ACKNOWLEDGMENT OF RISK AND WAIVER OF LIABILITY 
In consideration for being permitted to enter and use the facilities, equipment, services, and 
training provided by Real Authentic Wellness, located at 325 W Huron, Chicago, IL, I 
acknowledge and agree to the following: 
1. Assumption of Risk 
I understand that participation in physical exercise, fitness programs, personal training, group 
classes, and the use of gym equipment involves inherent risks including, but not limited to: 
● Muscle soreness 
● Sprains and strains 
● Overexertion 
● Equipment failure 
● Slips, falls, or other injuries 
I voluntarily choose to participate in such activities and assume full responsibility for any and 
all risks, injuries, or damages that may occur. 
2. Medical Clearance 
I represent that I am physically able to engage in physical exercise and have either: 
● Been cleared by a medical professional, or 
● Have chosen to participate at my own risk. 
I understand it is my responsibility to consult a physician prior to beginning any exercise or 
training program. 
3. Release of Liability 
I hereby release, waive, and discharge Real Authentic Wellness, its owners, employees, 
contractors, trainers, agents, affiliates, and volunteers from any and all liability, claims, 
demands, or causes of action arising out of or related to any injury, illness, loss, or damage 
sustained while on the premises or during participation in any gym-related activity—even if 
arising from negligence. 
4. Indemnification 
I agree to indemnify and hold harmless Real Authentic Wellness and its representatives from 
any claims, damages, costs, or expenses (including attorney’s fees) arising from my 
participation or conduct. 
5. Photography & Media Release  
I grant Real Authentic Wellness permission to use photos or videos taken of me during workouts 
or events for promotional and marketing purposes.

6. COVID-19 and Infectious Disease Acknowledgment 
I acknowledge that despite safety precautions, exposure to contagious diseases (including 
COVID-19) is possible. I accept this risk and release Real Authentic Wellness from liability in the

7. Acknowledgment of Understanding 
I have read this waiver thoroughly and understand its terms. By signing below, I confirm that I 
am waiving substantial rights, including the right to sue. This release shall be binding upon me,
