PARTICIPANT LIABILITY WAIVER & RELEASE

By signing this waiver, I agree to the following terms as a condition of participating in any HYLIFE group fitness class:

DESCRIPTION OF ACTIVITY. HYLIFE offers high-intensity group fitness classes that may include cardiovascular exercise, strength training, interval training, bodyweight movements, and other physically demanding activities.

ASSUMPTION OF RISK. I acknowledge that participation in high-intensity fitness classes carries inherent physical risks including muscular strain, joint injury, cardiovascular stress, falls, and other adverse physical outcomes. I voluntarily and knowingly assume all such risks associated with participation in HYLIFE classes.

MEDICAL CLEARANCE. I represent that I am in good physical health and have no known medical condition, injury, or physical limitation that would prevent safe participation. I have either consulted a licensed physician prior to participating or knowingly waive that opportunity and assume full personal responsibility for that decision. I agree to inform the instructor of any relevant injury or medical condition before each class.

RELEASE OF LIABILITY. To the fullest extent permitted by law, I release, waive, and hold harmless HYLIFE, Omega Wellness Club, their owners, instructors, employees, agents, and representatives from any and all claims, damages, losses, or liability of any kind arising out of my participation in any HYLIFE class, including personal injury, illness, or property damage. This release applies to claims arising from ordinary negligence and does not apply to gross negligence or willful misconduct.

INSTRUCTOR AUTHORITY. I agree to follow all HYLIFE class rules, safety guidelines, and instructor directions at all times. HYLIFE instructors reserve the right to remove any participant from a class at their discretion for safety or conduct reasons.

PHOTOGRAPHY & MEDIA. I grant HYLIFE a royalty-free license to use my likeness in connection with HYLIFE marketing and social media. I may opt out by notifying the instructor before class.

GOVERNING LAW. This waiver is governed by the laws of the State of Florida. Any dispute shall be brought exclusively in the courts of Miami-Dade County, Florida.

By completing registration and signing below, I confirm I have read, understood, and voluntarily agreed to all terms of this waiver. I am 18 years of age or older.
