TRAINER RESPONSIBILITIES: 
_X__ Will provide a preliminary fitness questionnaire. 
_X__ Will create exercise programming and provide supervision as a health coach
_X__ Will provide a safe and clean environment, clear of obstructions
_X__ Will monitor form and technique to ensure patrons are completing movements correctly



CLIENT RESPONSIBILITIES: 
____ Will promptly inform the trainer of any change in medical condition or of any new injury
____ Agrees to allow the Health Coach to contact a personal physician and/or obtain medical records when/if necessary
____ Will adhere to any policy, procedures and recommendations of the health coach/trainer 

Remember, exercise and balanced nutrition are EQUALLY important! During your fitness program, every effort will be made to assure your safety and enjoyment. However, as with any exercise program, there are risks, including increased heart stress and the chance of musculoskeletal injuries. In volunteering for this program, you agree to assume responsibility for these risks and waive any possibility for personal damage. You also agree that, to your knowledge, you have no limiting physical conditions or disability that would preclude a fitness program or participation in strenuous activities.  By signing below, you accept full responsibility for your own health and well-being AND you acknowledge an understanding that no responsibility is assumed by the leaders of the fitness program                       


We have read and understand the above training agreement. All of our questions have been answered and we both agree to abide by the above terms and conditions and to use our best efforts to meet our training goals. Before and after pictures will be taken and posted on the website (chicagobodyshop.com) for advertising and encouragement purposes.
