PILATES SCULPT EVENT LIABILITY WAIVER & RELEASE

1. ASSUMPTION OF RISK
I acknowledge that participating in fitness activities, including Pilates sculpt classes, involves inherent
risks. These risks may include, but are not limited to, muscle strain, injury, illness, or other physical
harm. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a
result of my participation.

2. HEALTH CONFIRMATION
I certify that I am physically able to participate in this class and have no medical condition that would
prevent my safe participation. I understand that it is my responsibility to consult with a physician prior to
engaging in any fitness program if I have concerns.

3. RELEASE OF LIABILITY
I hereby release, waive, and discharge Saturday Sculpt Club, event organizers, instructors, partners,
sponsors, and the venue from any and all liability, claims, demands, or causes of action arising out of or
related to any loss, damage, or injury that may be sustained while participating in the event.

4. PERSONAL RESPONSIBILITY
I agree to listen to my body, modify exercises as needed, and stop participation if I experience pain,
dizziness, or discomfort. I understand that instructors may provide guidance, but I am responsible for
my own safety.

5. PHOTO & VIDEO RELEASE
I grant permission for photos and videos taken during the event to be used for marketing, promotional,
and social media purposes without compensation, unless I notify the host in writing prior to the event.

6. AGREEMENT & SIGNATURE
By purchasing a ticket and/or signing below, I acknowledge that I have read, understood, and agree to
this waiver in full.
