Liability Waiver & Media Release**

I acknowledge that I am voluntarily participating in dance classes, workshops, events, and related activities hosted by Sparc Lab (“the Company”). I understand that participation in physical activity, including but not limited to dance, conditioning, stretching, and movement-based exercises, involves inherent risks that may result in injury.

Assumption of Risk

I knowingly and freely assume all risks, known and unknown, associated with participation in Sparc Lab activities, including but not limited to muscle strains, sprains, falls, collisions, illness, or other injuries that may occur during or after class.

Release of Liability

I hereby release, waive, discharge, and hold harmless Sparc Lab, its owners, directors, instructors, employees, contractors, volunteers, affiliates, venue partners, and sponsors from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage sustained while participating in Sparc Lab activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Medical Acknowledgment

I certify that I am physically able to participate in dance classes and have no medical conditions that would prevent my safe participation. I understand that Sparc Lab does not provide medical advice or diagnosis and that I am responsible for consulting a physician if needed.

Emergency Medical Treatment

In the event of an emergency, I authorize Sparc Lab to obtain medical treatment on my behalf if necessary and agree that I am financially responsible for any medical care received.

Media Release

I grant Sparc Lab the irrevocable right and permission to photograph, record, and use my image, likeness, voice, and/or performance in photos, videos, or other media taken during classes or events for marketing, promotional, educational, or commercial purposes, including but not limited to social media, website content, and advertisements. I waive any right to compensation or approval of such materials.

Minor Participants

If the participant is under 18 years of age, a parent or legal guardian must sign below, acknowledging and agreeing to all terms on behalf of the minor.

Acknowledgment & Agreement

I have read this waiver carefully, fully understand its terms, and understand that by signing it, I am giving up substantial rights, including the right to sue. I sign this agreement freely and voluntarily.
