Liability Waiver & Release Form

Mommy & Me Pilates at Social Blends Café

By purchasing a ticket and participating in this Mommy & Me Pilates class, I understand and agree to the following:

1. Voluntary Participation
I am voluntarily participating in this Mommy & Me Pilates class offered by Maria Rodriguez at Social Blends Café.

2. Assumption of Risk
I acknowledge that participating in exercise and physical activity involves inherent risks, including but not limited to muscle strains, sprains, falls, abnormal blood pressure, dizziness, fainting, heart-related conditions, or other injuries. I knowingly and voluntarily assume all risks associated with participating in this class for both myself and my child while attending the event.

3. Medical Clearance
I affirm that I am in good physical condition and do not suffer from any condition that would prevent or limit my safe participation. I have consulted with my physician regarding any medical concerns, including pregnancy or postpartum recovery, and have been medically cleared to participate in exercise. I understand that I am responsible for exercising within my own limits.

4. Child Supervision
I understand that I am solely responsible for the care, supervision, and safety of my child at all times before, during, and after class. I acknowledge that my child remains under my supervision throughout the event.

5. Release of Liability
I hereby release and discharge Maria Rodriguez, Social Blends Café, the property owners, event hosts, sponsors, and any affiliated staff or volunteers from any and all liability, claims, demands, or causes of action arising out of or related to my participation in this class or my child’s attendance, except in cases of gross negligence or willful misconduct.

6. Photography & Media Release
I understand that photos and videos may be taken during the event for promotional and marketing purposes. By attending, I consent to the use of my image and my child’s image unless I notify the instructor before class begins that I do not wish to be photographed or recorded.

7. Refund Policy
Refund requests will be considered on a case-by-case basis at the discretion of the event organizer.

8. Acknowledgement
By purchasing a ticket, I acknowledge that I have carefully read and fully understand this Liability Waiver & Release Form. I voluntarily agree to its terms and understand that my purchase serves as my electronic acceptance of this waiver.
