Outdoor Community Event Liability Waiver & Release Form
Participation Age Requirement
I acknowledge that BODY Hot Pilates requires all participants to be at least 16 years of age to
participate in outdoor classes or community fitness events. Participants under the age of 18 must have
a parent or legal guardian review and sign this waiver prior to participation. I confirm that I meet this age
requirement or, if under 18, that my parent or legal guardian has signed below to allow my participation.
Acknowledgment of Risk
I understand and acknowledge that participation in outdoor Pilates classes and community fitness
events hosted by BODY Hot Pilates involves physical activity conducted in an open, public, outdoor
environment. I acknowledge that outdoor fitness activities may involve risks including but not limited to
exposure to heat, sun, humidity, wind, changing weather conditions, dehydration, heat exhaustion,
dizziness, uneven surfaces, slippery ground, sand, grass, pavement, or other terrain hazards, and
potential physical injuries such as muscle strain, joint injury, falls, or other bodily harm.
Pregnancy Acknowledgment
I acknowledge and agree that participation in outdoor fitness activities may pose additional risks during
pregnancy. I confirm that I am not currently pregnant or, if I am pregnant, that I have received medical
clearance from my physician to participate. I understand that I am solely responsible for monitoring my
condition and modifying or discontinuing participation as needed.
Assumption of Risk
By signing this waiver, I voluntarily assume full responsibility for any risks, injuries, or damages—known
or unknown—that may occur as a result of my participation in BODY Hot Pilates outdoor classes or
community events. I understand the importance of hydration, sun protection, pacing, and
self-monitoring and agree to stop participating immediately if I feel unwell or unsafe.
Release of Liability
I hereby release, waive, and discharge BODY Hot Pilates, TMG, Aventura Mall Venture, and Boulevard
Step Ventures LLC, and their respective owners, instructors, employees, contractors, volunteers,
agents, affiliates, event partners, sponsors, and venue hosts (collectively, the 'Released Parties') from
any and all liability, claims, demands, actions, or causes of action arising out of or related to my
participation in any outdoor class or community event, whether caused by negligence or otherwise, to
the fullest extent permitted by law.
Indemnification
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs
(including attorney’s fees) incurred as a result of my participation in BODY Hot Pilates outdoor or
community-based activities.
