Pilates

I, the undersigned participant, understand that participation in Pilates classes involves physical activity that may include stretching, balance work, strength training, and other exercises that can place stress on the body. I understand that participation carries certain inherent risks of injury, including but not limited to muscle strain, sprains, falls, or other physical injuries.

I acknowledge that it is my responsibility to consult with a qualified physician before beginning any exercise program and to determine whether I am in good health and able to participate safely. I agree to follow all instructions carefully and to notify the instructor immediately if I experience pain, dizziness, or discomfort during class.

I voluntarily participate in this Pilates class at my own risk and assume full responsibility for any and all injuries or damages, whether caused by negligence or otherwise.

Waiver and Release of Liability

In consideration of being allowed to participate in Pilates classes conducted by Simply Graceful Pilates, I hereby release, waive, and discharge Simply Graceful Pilates, its owners, employees, independent contractors, and agents from any and all claims, demands, damages, or causes of action that may arise out of or relate to my participation in the class or use of the studio facilities and equipment.

This waiver and release includes, but is not limited to, any claims arising from negligence, accidents, equipment malfunction, or acts of other participants.

Health and Safety Disclosure

I affirm that I have disclosed any relevant medical conditions, injuries, or limitations that may affect my ability to participate safely. I agree to modify exercises as needed and understand that the instructor may provide adjustments, verbal cues, or demonstrations, but it is ultimately my responsibility to exercise within my own limits.
