Project Serve Beach – Injury and Accident Waiver 2025
Waiver and Medical Release Form
I will participate in training sessions, tryouts, open play, leagues, tournaments, clinics, camps,
and any other volleyball-related activities (the “Activities”) at Project Serve Beach.
I understand that participating in these Activities involves inherent risks, including but not
limited to injury, illness, or death due to factors such as playing conditions, weather, court
surfaces, equipment, and interactions with other participants.
I voluntarily assume all risks associated with participation and, on behalf of myself, my heirs,
and representatives, I fully and forever release, discharge, indemnify, and hold harmless
Project Serve Beach, its owners, coaches, staff, partners, sponsors, volunteers, and affiliated
organizations (“Releasees”) from any claims, damages, or legal actions arising from participation
in the Activities, whether known or unknown, anticipated or unanticipated.
Acknowledgment of Risks & Responsibilities
I, the undersigned participant, acknowledge and agree that:
1. Voluntary Participation – I am choosing to participate in sand volleyball and
understand that the sport involves risks such as running, jumping, diving, collisions, and
exposure to natural elements, all of which may result in injury.
2. Playing Conditions & Safety – I will inspect the playing surface and surrounding areas
before participating. If I choose to play, I accept the court's condition and waive any
claims related to defects or hazards.
3. Photography & Media Release – I understand that photos and videos (including live
streams) may be taken during Activities for promotional, educational, and entertainment
purposes. I grant Project Serve Beach permission to use my likeness in any media.
4. No Refunds for Rule Violations – I understand that violating Project Serve’s rules and
regulations may result in my removal from the event without a refund.
Medical Authorization & Infectious Disease Waiver
● I authorize any necessary medical treatment required while participating in these
Activities. I will inform Project Serve staff of any medical conditions or needs before
participation.
● I acknowledge the risk of contracting infectious diseases, including but not limited to
COVID-19. I understand that Project Serve Beach and its affiliates are not liable for any
illness resulting from participation.
● I release and waive any claims against Project Serve Beach and all related parties for
injuries, illnesses, or damages, whether caused by negligence or otherwise.
