Group Event Participant Waiver and Consent
This Waiver and Consent for Participation in Group Events (this “Waiver”) is executed by the
individual identified below (“Participant”), in connection with Participant’s participation in a
group fitness training event described below (the “Event”) hosted by The AMB Method LLC, a
New York limited liability company (the “Company”).

Description of the Event and Services
The Event consists of one-time participation in a group event provided by the Company. The
Event may take place at a location or space designated by the Company, which may vary by event
and may include indoor or outdoor spaces not owned or controlled by the Company.
The services provided at the Event are limited to personal training and general group fitness
instruction, and may include wellness workshops, workout classes, and other movement
experiences (the “Services”). The Services do not include, and must not be construed to include,
physical therapy, occupational therapy, medical treatment, or any other clinical or licensed
healthcare service. Any instructor at the Event who holds a physical therapy license or any other
healthcare license is acting solely in the capacity of a fitness instructor for purposes of the Event,
and no provider-patient relationship of any kind is created between Participant and the Company
or any instructor by virtue of Participant’s attendance at the Event.
Assumption of Risk
Participant acknowledges that the Event involves physical exercise and physical exertion,
including movements that may involve cardiovascular strain, resistance training, stretching, and
similar fitness activities. Participant acknowledges that these activities carry an inherent risk of
injury, including but not limited to muscle strain, sprain, joint injury, cardiovascular events, and,
in rare cases, serious injury or death.
Participant voluntarily and knowingly assumes all risks associated with participation in the Event,
whether known or unknown, and whether arising from the ordinary negligence of the Company
or otherwise, except as otherwise limited by this Waiver.
Release and Waiver of Liability
The Company does not warrant or guarantee any particular outcome, result, or level of
effectiveness from the Services. To the fullest extent permitted by law, the Company expressly
disclaims any and all liability for Participant’s decisions, actions, omissions, or Participant’s use 
of any movements, techniques, or methods following completion of the Event, including any
claims arising from injury, loss, damage, or adverse outcomes affecting Participants or any third
parties Participant may encounter. Participant assumes full responsibility for evaluating the
appropriateness of any movement, technique, or method for Participant’s own body and health
conditions.
To the fullest extent permitted by applicable law, Participant releases, waives, and discharges the
Company, and its owners, members, employees, and instructors, from any and all claims,
demands, or causes of action arising out of or related to Participant’s participation in the Event,
including claims based on the ordinary negligence of the Company.
This release does not waive, and must not be construed to waive, any claim arising from the gross
negligence, recklessness, or intentional misconduct of the Company, to the extent such a waiver
is prohibited by applicable law.
Indemnification
Participant agrees to indemnify and hold harmless the Company from any loss, liability, damage,
or cost the Company incurs due to Participant’s participation in the Event, except to the extent
arising from the Company’s gross negligence, recklessness, or intentional misconduct.
Physical Condition Acknowledgment
Participant represents that Participant has no medical condition that would prevent safe
participation in the Event, or has consulted a physician regarding participation and has been
cleared to participate. Participant is solely responsible for determining whether Participant is
physically able to participate in the Event.
Miscellaneous
This Waiver is governed by the laws of the State of New York, without regard to its conflict of
law principles.
Company retains the right to refuse participation in the Event for any reason. Participant shall
maintain civil conduct during Services and shall not act in manner that is disruptive or presents a
danger to the Company, the Company’s owners, employees, contractors, or agents, or other
Participants. In the event Participant does not maintain civil conduct, Company may remove or
initiate the removal of Participant or cancel or reschedule Services.
If any provision of this Waiver is held unenforceable, the remaining provisions will remain in full
force and effect, and the unenforceable provision must be modified to the minimum extent
necessary to make it enforceable.
This Waiver constitutes the entire agreement between Participant and the Company regarding the
Event and supersedes all prior or contemporaneous understandings regarding the Event.
Acknowledgment
Participant acknowledges that they have read this Waiver in its entirety, understands its terms,
have been provided material information regarding the Event, and sign it voluntarily as
Participant’s own free act.
This document may be electronically signed. Electronic signatures on this agreement are the
same as handwritten signatures for validity, enforceability, and admissibility purposes.
