Informed Consent and Liability Waiver
Anointed Strength is a faith-based fitness program operated by Flowing in Faith, LLC
I am voluntarily participating in an exercise-and-fitness program conducted by Flowing In Faith, LLC, doing business as Anointed Strength. I recognize that the program requires physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
I understand that, before participating in this program, it is my responsibility to consult with a physician to ensure I am medically able to engage in the physical activities this program entails. I represent and warrant that I have no medical condition that prevents me from participating in this program.
In consideration of participating in this program—including entering any premises, using any equipment, and engaging in the activities this program entails—I agree as follows:
•	I assume full responsibility for any risks, injuries, or damage known or unknown, which I might incur from participating in the program. Such injuries may include heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness, including death. This list is not exhaustive but illustrates my understanding that I’m responsible for any risks, injuries, or damage I may incur from participating in the program.
•	I accept that Flowing In Faith, LLC (including its agents, members, managers, officers, and owners) assumes no duty to me to ensure my physical or medical ability to participate in any physical activity the program involves.
•	I knowingly, voluntarily, and expressly waive any claim I may have against Flowing in Faith, LLC (including its agents, members, managers, officers, and owners) for any injury or damages that I may sustain from participating in the program.
•	I, my heirs, and my representatives, covenant not to sue and forever release, waive, discharge any claims and liabilities against Flowing in Faith, LLC (including its agents, members, managers, officers, and owners) arising from any injury or death caused by any negligence or other acts.
•	I allow myself to be filmed and photographed and publicly shared for security, marketing, and promotional purposes.
•	Any dispute between me and Flowing In Faith, LLC (including its agents, members, managers, officers, and owners) that arises from my participation in the program must be adjudicated in Travis County under Texas law.
•	If any provision of this agreement is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall remain in full force and effect.

I have read the above consent, waiver, and release of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
