PLEASE READ CAREFULLY. BY SIGNING THIS DOCUMENT, YOU ACKNOWLEDGE AND AGREE TO THE FOLLOWING:

I, the undersigned participant, acknowledge that I am voluntarily participating in Somatic Breathwork and Sound Healing at Yoga Vida. I understand that breathwork and sound healing involve deep breathing, movement, and exposure to vibrational frequencies that can bring about emotional and physical responses. I acknowledge that these practices can be physically, mentally, and emotionally intense and that I am responsible for my own well-being during the session.

CONTRAINDICATIONS

I understand that somatic breathwork may not be suitable for individuals with certain medical conditions, including but not limited to:

Pregnancy

Cardiovascular disease, high blood pressure, or history of heart attacks

Epilepsy or a history of seizures

Glaucoma or retinal detachment

Severe mental illness or history of psychosis

Osteoporosis or recent fractures

Recent surgery or significant physical injuries

I affirm that I do not have any of the above conditions, or if I do, I have consulted with my healthcare provider and have been cleared to participate. I acknowledge that I am solely responsible for my health and safety and will listen to my body’s signals throughout the session.

ASSUMPTION OF RISK

I acknowledge that participation in breathwork and sound healing carries potential risks, including but not limited to dizziness, lightheadedness, emotional release, altered states of consciousness, and physical discomfort. I voluntarily assume all risks associated with this practice and agree to take full responsibility for my participation.

RELEASE OF LIABILITY

I hereby release and discharge Yoga Vida, the event facilitators, instructors, volunteers, Eleanor Rome, Annie Bosco, and any affiliated individuals or organizations from any and all liability, claims, demands, or causes of action that may arise from my participation. This release includes but is not limited to injuries, psychological distress, or any other consequences that may result from my engagement in the session.

INFORMED CONSENT

I confirm that I have been informed about the nature of somatic breathwork and sound healing, including its potential effects, and I am choosing to participate of my own free will. I understand that this practice is not a substitute for medical or psychological care, and I will seek professional advice if needed.
