Participation Waiver, Assumption of Risk, and Media Release
Effective Upon Ticket Registration

By reserving a ticket for any class, event, or session with Level Up Mobile Gym, LLC, I acknowledge, understand, and agree to the following:

1. Voluntary Participation & Assumption of Risk

I understand that physical fitness training, including but not limited to strength training, high-intensity exercise, cardio, and bodyweight movements, involves risks of injury, illness, or even death. By registering, I voluntarily assume full responsibility for any and all risks — known or unknown — that may arise from participation.

2. Waiver and Release of Liability

In consideration for being permitted to participate in a workout or event hosted by Level Up Mobile Gym, LLC, I, for myself and my heirs, executors, and assigns, do hereby release and hold harmless Level Up Mobile Gym, LLC, its owners, trainers, staff, contractors, affiliates, event partners, and property owners from any and all claims or causes of action resulting from injury, illness, or damages related to participation.

This release applies whether the activity takes place on public or private property, outdoors, indoors, or in transit.

3. Health Acknowledgment

I confirm that I am in good health, have no medical or physical condition that would prevent me from safely participating, and have been advised to consult with a physician before beginning any new fitness activity.

If I experience any injury or distress during a workout, I will immediately inform a trainer and stop participation.

4. Emergency Authorization

In the case of emergency, I authorize Level Up Mobile Gym, LLC and its staff to seek emergency medical care on my behalf. I understand that I am responsible for any resulting medical expenses.

5. Media and Social Media Release

I understand that photos and videos may be taken during classes and events. By reserving a ticket, I grant Level Up Mobile Gym, LLC full rights to use these images and recordings — including my likeness — for promotional purposes, including social media, advertising, and digital content, with no compensation owed.
