By signing up for and/or participating in any class, event, or activity offered by Movement with Sadie (MWS), I acknowledge, agree, and represent that:
	1.	Voluntary Participation: I am voluntarily participating in a mat Pilates class or event offered by MWS. I understand the nature of physical activity and acknowledge that participation in such classes involves inherent risks including, but not limited to, physical injury, illness, muscle strain, or other unforeseen complications.
	2.	Health Certification: I certify that I am in good physical condition and do not suffer from any condition, impairment, or disability that would prevent or limit my participation in the event. I will notify the instructor of any pre-existing medical conditions or injuries before beginning class.
	3.	Assumption of Risk: I understand and agree that MWS, its instructors, staff, and affiliates shall not be held liable for any injuries or damages resulting from my participation. I assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in the class or event.
	4.	Release of Liability: I hereby waive, release, and discharge MWS, its instructors, employees, volunteers, and representatives from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the class or event.
	5.	Photography/Media Consent (Optional): I understand that photos or videos may be taken during the class or event for marketing or promotional purposes. I grant permission for MWS to use my image unless I opt out by notifying the instructor in writing prior to the event.

By registering for a Movement with Sadie class or event, I confirm that I have read, understood, and agree to the above terms.

If participant is under 18, a parent or legal guardian must complete this form on their behalf.
