Participant Information
Assumption of Risk, Waiver, and Release of Liability
I understand that participation in Belly Heat classes involves physical activity including, but not limited to, dance, stretching, cardio, and fitness movements. I acknowledge that these activities carry certain inherent risks, including the risk of injury, illness, or aggravation of pre-existing conditions.
By signing below:
I voluntarily assume all risks associated with participating in Belly Heat classes, whether occurring before, during, or after the class.
I certify that I am in good physical health and have consulted with my physician if I have any medical concerns about engaging in exercise.
I release, indemnify, and hold harmless Belly Heat, its instructors, staff, affiliates, and representatives from any and all liability, claims, or demands that may arise out of my participation in these classes.
I understand that I am solely responsible for my personal belongings during class.
Media Release
I grant Belly Heat and its representatives permission to capture photos, video, and audio recordings of me during classes or related events. I understand these may be used for promotional purposes including, but not limited to:
Social media content (Instagram, Facebook, TikTok, YouTube, Pinterest, etc.)
Marketing and advertising materials (digital or print)
Website or class promotions
I agree that these images and recordings may be edited, published, and distributed without restriction, and I waive any right to royalties, compensation, or approval of the final products.
If I do not wish to be photographed or recorded, I will notify the instructor prior to the start of class.
Acknowledgment and Signature
I have read, understood, and voluntarily agree to the terms of this waiver and media release.
