**What to expect:** Houston’s HOTTEST Mat Pilates class featuring a mix of Rauw Alejandro and Benito. A 45- minute hot mat Pilates class featuring music using weights, bands & more.


**What to bring:** Water, sweat towel, **<u>yoga mat</u>** and the BEST version of yourself. Grip socks are optional!

I, the Participant, affirm that I am in good physical condition and do not suffer from any condition that
would prevent or limit my participation in training with a BodyByMuika instructor.

I confirm that I have no health issues (including but not limited to: cardiac irregularities; spinal, bone,
joint, tendon, or ligament injuries; dizziness; asthma or other breathing difficulties; diabetes; epilepsy; or
allergies) that may affect my participation in any BodyByMuika session. Participants with low or high
blood pressure and/or cardiac irregularities should not attend class without medical clearance.

Medical Recommendations
Prior to engaging in any BodyByMuika activity, you should consult your physician and receive a
physical exam, especially if you have a history of heart disease, arthritis, spinal pain or trauma,
diabetes, high blood pressure, or anemia.

BodyByMuika does not provide medical treatment or medical advice. Instructors may share general
fitness guidance, but this should not replace medical consultation. Always follow the advice of your
licensed healthcare provider.

Assumption of Risk
I understand that I am voluntarily participating in strenuous physical activity, including but not limited to
Pilates on the reformer, mat work, and/or fitness equipment (weights, sliders, bands). I acknowledge
that these activities involve inherent risks, including but not limited to: elevated heart rate, abnormal
blood pressure, fainting, heart attack, injury, or death.
I assume full responsibility for my participation and agree to release and hold harmless BodyByMuika,
its instructors, independent contractors, and participants from any liability, injury, loss, or damage I may
suffer, including death, as a result of my participation.

Liability Release
By signing this agreement, enrolling online, or attending any BodyByMuika class, event, or program (in
person or online), I acknowledge and agree on behalf of myself, my heirs, personal representatives,
and assigns that:
1. I accept all risks associated with participation.
2. I release BodyByMuika and its representatives from all claims or liabilities arising out of my
participation.
3. I accept responsibility for my own safety and well-being during all activities.

MinorsNo one under the age of 16 may participate. Minors ages 16–18 may participate only with a parent or
legal guardian’s written consent.

Pregnancy
If pregnant, I acknowledge that there are additional risks involved. I understand that I should consult
with my doctor before participating in BodyByMuika activities.

COVID-19 Addendum
BodyByMuika prioritizes the safety of its community. By attending classes or events, I acknowledge
that:
- BodyByMuika takes reasonable precautions (including cleaning and disinfecting equipment) but
cannot guarantee protection from COVID-19 or other illnesses.
- If I am feeling sick, have a fever, or show flu-like symptoms, I will not attend class until cleared.
- I voluntarily assume the risk of exposure to COVID-19 or other communicable diseases by
participating in BodyByMuika activities and release BodyByMuika and its community members from
liability.
