This run is an opportunity to put his story in front of the community, give him the spotlight he deserves & help him find a home🏡



**Participants will start and finish at Juice It Up!** on La Sierra, enjoying a relaxed, 2 mile experience. Whether you run or walk, every step brings awareness to dogs like Bingo.



**Admission includes:**
A custom event t-shirt featuring Bingo
A 20 oz smoothie from Juice It Up!
Entry into a meaningful run supporting local shelter dogs

BINGO’S SMOOTHIE RUN – LIABILITY WAIVER & RELEASE

Event: Bingo’s Smoothie Run
Location: Juice It Up! La Sierra, Riverside, CA
Organizations: Jonis Allen Foundation, Juice It Up!, Yucaipa Animal Placement Society (YAPS)

Assumption of Risk

I acknowledge that participating in Bingo’s Smoothie Run, including running, walking, and being around animals, involves inherent risks. These risks may include, but are not limited to:

Physical injury, illness, or death
Slips, trips, falls, or collisions
Heat-related illness or dehydration
Interaction with animals, including bites, scratches, or unpredictable behavior
Hazards related to outdoor conditions, traffic, or event equipment

I voluntarily assume all risks associated with participation.

Release of Liability

In consideration of being allowed to participate, I hereby release, waive, and discharge:

The Jonis Allen Foundation, Bingo’s Smoothie Run, Juice It Up!, Yucaipa Animal Placement Society (YAPS), and all of their respective owners, officers, directors, employees, volunteers, sponsors, partners, and affiliates

from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me, whether caused by negligence or otherwise.

Animal Interaction Acknowledgment

I understand that animals present at the event may be unpredictable. I agree to:

Interact with animals at my own risk
Follow all instructions from event staff and shelter personnel
Not approach or handle any animal without permission

I assume full responsibility for any injuries or damages resulting from animal interactions.

Medical Authorization

I certify that I am physically able to participate in this event. In the event of an emergency, I authorize event staff to obtain medical treatment on my behalf if I am unable to do so. I understand that I am responsible for any medical costs incurred.

Indemnification

I agree to indemnify and hold harmless the Jonis Allen Foundation, Bingo’s Smoothie Run, Juice It Up!, and YAPS from any claims, damages, or expenses arising from my participation in the event.

Photo & Media Release

I grant permission to the Jonis Allen Foundation and its partners to use any photographs, videos, or recordings of me taken during the event for marketing, promotional, and media purposes without compensation.

Rules & Conduct

I agree to follow all event rules, including:

Obeying staff and volunteer instructions
Maintaining control of any dog I bring (leash required at all times)
Acting responsibly and respectfully toward others and animals

Failure to comply may result in removal from the event without refund.

Acknowledgment & Agreement

By submitting this waiver, I acknowledge that I have read, understood, and voluntarily agree to this waiver and release of liability.
