GENERAL LIABILITY WAIVER AND RELEASE OF LIABILITY
SoFlo Wellness Collective, Ice Lounge Wellness, and Partnered Events
PARTICIPANT AGREEMENT, RELEASE, AND ASSUMPTION OF RISK
I, the undersigned, acknowledge that I am voluntarily participating in an event, activity, or program (hereinafter referred to as "the Event") hosted by SoFlo Wellness Collective, Ice Lounge Wellness, or any of their partners (hereinafter referred to as "the Hosts"). I understand and acknowledge that participation in such events may include but is not limited to: fitness activities, running, physical exertion, strength training, yoga, stretching, cold water immersion, and consumption of food or beverages, including alcoholic beverages. I recognize that these activities carry inherent risks, including but not limited to: slips, trips, falls, dehydration, overexertion, cold shock, hypothermia, injury, illness, allergic reactions, or other potential hazards associated with participation.
ASSUMPTION OF RISK
I hereby acknowledge and assume all risks associated with my participation in the Event, including but not limited to those described above. I understand that it is my responsibility to ensure that I am in good physical condition to participate and that I have consulted with a medical professional if I have any concerns about my health or ability to participate safely.
WAIVER AND RELEASE
I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby waive, release, and discharge the Hosts, their owners, employees, volunteers, sponsors, affiliates, and any other related parties from any and all claims, liabilities, demands, or causes of action that may arise as a result of my participation in the Event, including but not limited to personal injury, property damage, illness, or wrongful death, whether caused by negligence or otherwise.
INDEMNIFICATION
I agree to indemnify and hold harmless the Hosts from any claims, losses, or damages that may arise from my participation in the Event, including attorney’s fees and legal expenses incurred in defense of any claims arising therefrom.
MEDICAL TREATMENT
I authorize the Hosts and their representatives to provide first aid or medical treatment in the event of an emergency and understand that I am responsible for any associated medical costs.
PHOTOGRAPHY/VIDEO RELEASE
I grant permission for the Hosts to use photographs, videos, or other media taken during the Event for promotional, marketing, or other business purposes without compensation.
ACKNOWLEDGEMENT OF ALCOHOL CONSUMPTION
I acknowledge that alcoholic beverages may be available at the Event. If I choose to consume alcohol, I do so at my own risk and assume full responsibility for my actions. I agree to drink responsibly and understand that the Hosts are not responsible for any consequences resulting from alcohol consumption.
SEVERABILITY
If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
ACKNOWLEDGEMENT AND ACCEPTANCE
I have read and fully understand this waiver and release of liability. I voluntarily agree to its terms and acknowledge that by signing this document, I am waiving legal rights, including the right to sue.
