DRIPS N CHILL: WAIVER & RELEASE

Event: Drips N Chill
Date: March 15, 2026
Released Parties: Drip Health Academy d/b/a Drips N Chill, and its owners, officers, employees, contractors, medical staff, volunteers, sponsors, vendors, venue(s), and affiliates (“Released Parties”).

1) RELEASE OF LIABILITY (INCLUDING NEGLIGENCE)

In consideration of access to the Event and all related activities (including fitness/dance classes, cold plunges/ice baths, sauna, breathwork/stretching, compression/recovery, vendor activations, optional IV therapies and vitamin injections, and general use of the premises), I release and discharge the Released Parties from any and all claims, demands, or liabilities for injury, illness, property damage, or death arising out of or related to my participation, EVEN IF CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE of any Released Party or any participant. This release does not waive claims based on gross negligence or intentional misconduct.

2) ASSUMPTION OF RISK

I understand and voluntarily accept the inherent risks of participating in the Event and its activities, including but not limited to:
• Fitness/dance risks: slips/falls, collisions, sprains/strains, overuse, twisting/turning, floorwork, light/sound exposure.
• Temperature/recovery risks: cold shock, hypothermia, overheating, burns.
• General risks: muscle/joint/cardiac events, dehydration, food allergies/foodborne illness, exposure to communicable diseases.
• IV/IM risks (if elected): infection, bruising, infiltration, allergic reaction, vasovagal response.
I certify I am fit to participate and will follow posted rules and staff instructions.

3) OPTIONAL MEDICAL TREATMENTS (IV/VITAMIN SHOTS)

If I choose to receive IV therapy or vitamin injections, I acknowledge the above risks, will disclose relevant medical history, and understand I may refuse or discontinue any treatment at any time.

4) INDEMNIFICATION

I agree to defend, indemnify, and hold harmless the Released Parties from any claim, demand, loss, or expense (including reasonable attorneys’ fees) arising from my participation or my breach of this agreement.

5) MINORS — PARENT/LEGAL GUARDIAN RESPONSIBILITY & FLORIDA NOTICE 

This Event is not designed for children. If I bring a minor, I understand the minor may attend only with continuous supervision by me (the parent/legal guardian). I am fully responsible for the minor’s safety, conduct, and any injury or damage caused by the minor. I agree that, to the maximum extent permitted by Florida law, I am releasing claims on behalf of the minor for risks that are a natural part of the activities, and the Released Parties are not responsible for injuries to the minor that arise from such inherent risks.


FLORIDA NOTICE TO THE MINOR CHILD’S NATURAL GUARDIAN:
READ THIS COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. EVEN IF DRIP HEALTH ACADEMY D/B/A DRIPS N CHILL USES REASONABLE CARE, YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BECAUSE CERTAIN DANGERS ARE INHERENT IN THE ACTIVITY. BY SIGNING THIS FORM, YOU ARE GIVING UP YOUR CHILD’S RIGHT, AND YOUR RIGHT, TO RECOVER FROM DRIP HEALTH ACADEMY D/B/A DRIPS N CHILL FOR ANY INJURY, INCLUDING DEATH, OR PROPERTY DAMAGE RESULTING FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU MAY REFUSE TO SIGN; DRIP HEALTH ACADEMY D/B/A DRIPS N CHILL MAY REFUSE YOUR CHILD’S PARTICIPATION IF YOU DO NOT.

By signing this waiver, I confirm I am the natural guardian of any listed minor(s); I accept all inherent risks on their behalf; I agree to supervise them at all times; and I acknowledge that minors do not sign this waiver.

6) PUBLICITY & CONTENT

I grant the Released Parties the right to use my image/voice in Event media for lawful promotional purposes without compensation. All Event media is the sole property of the Released Parties.

7) MISCELLANEOUS

Florida law governs. If any provision is held invalid, the remainder remains effective. Electronic consent has the same effect as a signed original.
