PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR
RIGHTS.
I, the undersigned, acknowledge that I have voluntarily chosen to participate in a Pilates class or event
organized by Perreo& Pilates, organized by CBC-Marketing and her team. I understand and agree to the
following:
1. Assumption of Risk
I understand that participating in Pilates involves physical exertion and may include exercises that
carry a risk of injury. I confirm that I am voluntarily participating and assume all risks associated
with this activity, including but not limited to falls, muscle strains, sprains, or other injuries.
2. Health and Medical Condition
I affirm that I am physically fit and have no medical conditions that would prevent my safe
participation. I agree to consult a physician before participating if I have any concerns. Before the
class begins, I will notify the instructor of any existing injuries or limitations.
3. Release of Liability
I hereby release, waive, and discharge Perreo & Pilates, CBC-Marketing, instructors, event
organizers, sponsors, venue owners, and all affiliates from any and all claims, liabilities,
demands, or causes of action related to any injury, illness, loss, or damage I may sustain as a
result of my participation.
4. Photography and Media Release
I grant Perreo & Pilates and CBC-Marketing, permission to use photos, videos, or recordings of
me taken during the event for promotional and marketing purposes without compensation.
5. Emergency Medical Treatment
In the event of an emergency, I authorize Perreo & Pilates, CBC-Marketing representatives to
obtain necessary medical treatment for me. I understand that I am responsible for any associated
medical costs.
6. Acknowledgment of Policies
I agree to follow all class guidelines and instructions from instructors to ensure a safe and
enjoyable experience for all participants.
By signing below, I acknowledge that I have read, understood, and agreed to the terms of this waiver. I
understand that I am giving up substantial rights by signing it and do so voluntarily.
