**LIABILITY WAIVER AND RELEASE OF CLAIMS (FLORIDA)**

By signing below, I acknowledge that I am voluntarily participating in Pilates classes, fitness activities, and related events offered by PraiseForm Pilates and its instructor(s), including but not limited to classes held at beaches, parks, studios, or other locations.

I understand that participation in physical exercise, including Pilates, involves inherent risks, including but not limited to muscle strain, injury, falls, illness, or other health complications. I voluntarily assume all risks, known or unknown, associated with my participation.

I affirm that I am in good physical condition and have no medical condition that would prevent me from safely participating. I understand that it is my responsibility to consult with a physician prior to participating if I have any concerns about my health.

In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless PraiseForm Pilates, its owner (Liz Nguyen), instructors, contractors, volunteers, and any affiliated entities from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in these activities, whether caused by negligence or otherwise, to the fullest extent permitted by Florida law.

I acknowledge that classes may take place outdoors, including at beaches and parks, and I assume all risks associated with outdoor environments, including uneven surfaces, weather conditions, and other natural hazards.

I consent to receive first aid or emergency medical treatment if deemed necessary during participation.

I grant permission to PraiseForm Pilates to use photographs or videos taken during classes for promotional purposes, unless I notify the instructor in writing prior to participation.

I have read this waiver carefully, fully understand its terms, and sign it freely and voluntarily. I understand that by signing this document, I am giving up certain legal rights.
