This is a complimentary wellness event for healthcare workers. To receive FREE entry, you must present a valid healthcare worker badge or healthcare student identification at check-in.

Because this is a free community event, we kindly ask that you only RSVP if you truly plan on attending. Every reserved ticket takes a spot from another healthcare worker who may genuinely want or need this experience.

If your plans change, no worries at all Please simply cancel your ticket or send us a message so we can open your spot to someone else in the community.

The Burnout to Balance Series is a community wellness initiative designed to promote stress reduction, healthy life balance, and community connection among healthcare professionals.

By signing below, I acknowledge and agree to the following:

Assumption of Risk

I understand that participation in boxing, fitness classes, exercise activities, stretching, warm-ups, demonstrations, and related wellness activities involves inherent risks, including but not limited to:

* Muscle strains, sprains, or soreness
* Falls or loss of balance
* Physical contact with equipment or other participants
* Increased heart rate and blood pressure
* Serious injury, disability, or, in rare circumstances, death

I voluntarily choose to participate and assume all risks associated with my participation.

Medical Acknowledgment

I certify that I am physically able to participate in this event and have no medical condition that would prevent safe participation. I understand it is my responsibility to consult a healthcare provider if I have concerns regarding my ability to participate.

Release of Liability

In consideration of being permitted to participate, I release and hold harmless:

Burnout to Balance Series, In Her Social Era, event organizers, volunteers, instructors, venue partners, sponsors, and affiliates from any and all claims, demands, causes of action, damages, losses, or liabilities arising from my participation in this event, except where prohibited by law.

Personal Responsibility

I agree to:

* Follow all instructions provided by event staff and instructors.
* Exercise within my personal limits and stop activity if I experience pain, dizziness, or discomfort.
* Use equipment safely and responsibly.

Photo & Video Release

I understand that photographs and videos may be taken during the event for promotional, educational, and social media purposes.
