Acknowledgment and Assumption of Risk:
I, the undersigned, hereby acknowledge that I have voluntarily elected to participate in the cold
plunge event organized by Templ Wellness. I understand that participation in a cold plunge
involves submersion in cold water, which can result in various physical reactions and risks,
including but not limited to:
• Cold shock response
• Hyperventilation
• Hypothermia
• Dizziness or light-headedness
• Increased heart rate
• Cardiac arrest

I acknowledge that the following health conditions are advised not to participate in a cold plunge
due to potential adverse effects:
• Cardiovascular disease
• High blood pressure
• Asthma
• Epilepsy or seizure disorders
• Diabetes
• Cold-induced asthma or cold urticaria
• Raynaud's disease
• Any condition that affects the circulatory system

Medical Clearance: I understand that it is my responsibility to consult with a physician before
participating in the cold plunge if I have any of the above conditions or any other health
concerns. I confirm that I am physically fit and have no medical conditions that would preclude
my participation in this activity.

Acknowledgment of Risks: I, the undersigned participant, acknowledge and agree to the
following:
1. Voluntary Participation: I am voluntarily participating in the Community Cold Plunge
event at Templ Wellness. I understand that this activity involves exposure to cold water,
which can pose various health risks.
2. Health and Safety Risks: I understand that participation in a cold plunge can cause
sudden changes in heart rate, blood pressure, and body temperature, which may result in
hypothermia, cardiac arrest, or other serious health conditions. I acknowledge that it is
my responsibility to consult with a healthcare provider before participating if I have any
pre-existing medical conditions or concerns.
3. Assumption of Risk: I assume all risks associated with my participation in the cold
plunge, including but not limited to physical injury, illness, or any other adverse effects
that may occur as a result of exposure to cold water.
4. Release of Liability: In consideration of being allowed to participate in this event, I
hereby release, waive, discharge, and covenant not to sue Templ Wellness, its owners,
employees, volunteers, or agents from any and all liability, claims, demands, actions, or
causes of action arising out of or related to any loss, damage, injury, or death that may be
sustained by me as a result of my participation in the cold plunge.
5. Medical Treatment: In the event of an emergency, I authorize Templ Wellness and its
representatives to seek medical treatment on my behalf. I understand that I will be
responsible for any medical expenses incurred as a result.
6. Photo and Video Release: I grant Templ Wellness permission to use any photographs,
videos, or other recordings taken during the event for promotional or educational
purposes.

Acknowledgment and Signature: I have read and fully understand the terms of this Liability
Waiver. I am signing it freely and voluntarily, intending to be legally bound by its terms.
