WELLNESS EVENT PARTICIPATION WAIVER & RELEASE OF LIABILITY

By participating in the Wellness Event, I acknowledge and agree that I am physically able and medically fit to participate in the exercise, movement, dance, mindfulness, and other physical activities included in the event.

I understand that participation in physical activity involves inherent risks, including but not limited to injury, illness, dizziness, falls, or other physical harm. I voluntarily choose to participate and assume all risks associated with my participation.

I agree to release and hold harmless the event organizers, Sip & Shine LLC, Herbalife, and their respective owners, employees, instructors, coaches, representatives, affiliates, and volunteers from any and all liability, claims, damages, or injuries that may arise from or be related to my participation in the Wellness Event, except where prohibited by law.

I understand that I should consult with a physician before participating in physical activity if I have any medical condition, health concern, or doubt about my ability to safely participate.

Please check with your physician before participating if any of the following apply to you:

• You have been prescribed medication for blood pressure or a heart condition.
• A physician has advised you to participate only in specific types of physical activity.
• You experience chest pain during physical activity.
• You have experienced chest pain within the past month while not participating in physical activity.
• You have lost your balance due to dizziness or have experienced unexplained fainting.
• You have a bone or joint problem (for example, involving your back, knee, or hip) that could worsen with physical activity.
• You are pregnant or have given birth within the past six months.
• There is any other reason why you believe you should not participate in physical activity.

I understand that if I experience dizziness, weakness, lightheadedness, nausea, chest pain, shortness of breath, or any other pain or discomfort during the event, I must stop participating immediately and seek appropriate medical attention.

I understand that participation in the Wellness Event is voluntary and that I am participating entirely at my own risk and responsibility.

PHOTO & VIDEO RELEASE

I grant the event organizers permission to photograph and/or record me during the Wellness Event. I understand that these photographs and videos may be used for promotional, marketing, social media, advertising, and other business purposes without additional compensation or prior approval.

By completing and submitting this waiver, I confirm that I have read, understood, and voluntarily agreed to all of the statements and terms contained in this document. I acknowledge that I have had the opportunity to ask questions and that I am choosing to participate in the Wellness Event voluntarily and at my own risk.
