Liability Waiver - C&C
1. Acknowledgment and Assumption of Risk
I, the undersigned parent/guardian, understand that participation in Core & Cradle Mommy & Me Pilates classes and events involves physical activity that may carry inherent risks to both myself and my child(ren). These risks may include, but are not limited to, muscle strain, falls, collisions with other participants or children, use of equipment, and conditions related to the physical environment or venue.
I acknowledge that I am voluntarily participating in this activity with full knowledge of these risks and agree to assume full responsibility for any injury, harm, or loss that may occur to myself or my child(ren).

2. Responsibility for Child(ren) and Supervision
I acknowledge and agree that I am solely responsible for the safety, supervision, and well-being of my child(ren) at all times during the class or event. I understand that Core & Cradle classes are not a childcare service, and that instructors, staff, and volunteers do not provide supervision for children.
I agree to remain present with my child(ren) at all times and to ensure that my child(ren) do not pose a risk to themselves or others.

3. Medical Clearance
I certify that I and my child(ren) are physically able to participate in this class/event and that I have consulted with a physician if necessary. I confirm that I have no medical conditions that would prevent safe participation and assume all risks associated with participation.

4. Release and Waiver of Liability
I hereby release, waive, discharge, and hold harmless Core & Cradle, its owners, instructors, employees, agents, volunteers, and affiliates from any and all claims, demands, causes of action, liabilities, damages, or expenses arising out of or in connection with my or my child(ren)’s participation in any Core & Cradle class or event, whether caused by the negligence of the releasees or otherwise.

5. Third-Party Venues
I understand that Core & Cradle classes and events may take place at various third-party locations, including but not limited to studios, parks, and partner venues. I hereby release and hold harmless the owners, operators, and staff of any venue where a Core & Cradle class or event is held from any claims or liabilities arising from participation.

6. Emergency Medical Authorization
In the event of an emergency, I authorize Core & Cradle to seek medical treatment for myself and/or my child(ren) if deemed necessary. I understand and agree that I am responsible for any costs associated with such medical treatment.

7. Photo and Video Consent
I grant permission for Core & Cradle to use photographs, videos, or other media recordings of myself and/or my child(ren) taken during the class or event for promotional, marketing, or social media purposes. I understand that I will not receive compensation for such use.

8. Governing Law
This waiver shall be governed by and interpreted in accordance with the laws of the State of Florida.

9. Severability
If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

I HAVE READ THIS WAIVER AND RELEASE, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY.
