CORE AND CRADLE RELEASE OF LIABILITY, ASSUMPTION OF RISK, PARTICIPANT AGREEMENT, MEDICAL AUTHORIZATION, AND MEDIA RELEASE

PLEASE READ CAREFULLY. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS.

By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Release of Liability, Assumption of Risk, Participant Agreement, Medical Authorization, and Media Release ("Agreement").
For purposes of this Agreement, "Core and Cradle" includes Core and Cradle LLC, its owners, members, managers, officers, employees, instructors, guest instructors, independent contractors, volunteers, affiliates, sponsors, vendors, exhibitors, event partners, venue owners, landlords, representatives, agents, successors, assigns, and any third-party service providers engaged in connection with Core and Cradle activities or events.

1. ACKNOWLEDGMENT AND ASSUMPTION OF RISK
I understand that participation in Core and Cradle classes, Pilates classes, workouts, fitness activities, wellness programs, educational programming, workshops, family events, community events, sponsored activations, recreational activities, vendor experiences, and related programming involves inherent risks.
These risks may include, but are not limited to:
• Physical exertion and exercise-related injuries • Muscle strains, sprains, tears, overexertion, and cardiovascular events • Slips, trips, falls, and collisions • Use of fitness equipment, props, mats, furniture, playground equipment, soft play equipment, inflatables, event installations, and other equipment • Injuries caused by participants, children, instructors, vendors, or third parties • Exposure to communicable illnesses and diseases • Exposure to weather, environmental conditions, insects, animals, and venue-related hazards • Risks associated with infants, toddlers, and children attending events • Risks associated with food, beverages, products, samples, demonstrations, and services provided at events • Risks arising from participation at third-party venues

I knowingly and voluntarily assume all risks, known and unknown, foreseeable and unforeseeable, associated with my participation and the participation of my child(ren).

2. RESPONSIBILITY FOR CHILDREN
I understand that Core and Cradle does not provide childcare, babysitting, child supervision, or child monitoring services.

I acknowledge and agree that I am solely responsible for the supervision, safety, conduct, behavior, and well-being of my child(ren) at all times while attending any Core and Cradle class, event, or activity.
I agree to remain present and actively supervise my child(ren) throughout the entirety of the event or activity.
I further acknowledge that Core and Cradle's instructors, guest instructors, volunteers, vendors, sponsors, and staff are not responsible for supervising my child(ren).

3. HEALTH, FITNESS, PREGNANCY, AND POSTPARTUM ACKNOWLEDGMENT
I certify that I am physically capable of participating in activities offered by Core and Cradle.
I understand that participation is voluntary and that I am solely responsible for determining whether I am physically and medically capable of participating in any activity offered by Core and Cradle.
If I am pregnant, postpartum, recently gave birth, recovering from a cesarean section, experiencing pelvic floor dysfunction, diastasis recti, abdominal separation, or any other medical condition, I certify that I have obtained any medical clearance I deem necessary before participating.

I understand that Core and Cradle instructors, guest instructors, and representatives are not physicians, healthcare providers, physical therapists, medical professionals, or rehabilitation specialists and do not provide medical advice, diagnosis, treatment, rehabilitation services, or healthcare recommendations.
I understand that exercise modifications, regressions, and progressions may be offered during classes and events.

I accept full responsibility for selecting, modifying, progressing, regressing, or declining any exercise, movement, activity, or instruction based upon my own physical condition, abilities, limitations, and comfort level.
I agree to immediately stop participation and seek appropriate medical attention if I experience pain, dizziness, bleeding, shortness of breath, discomfort, contractions, injury, or any other concerning symptoms.

4. RELEASE OF LIABILITY
TO THE FULLEST EXTENT PERMITTED BY LAW, I RELEASE, WAIVE, DISCHARGE, AND HOLD HARMLESS CORE AND CRADLE FROM ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION, DAMAGES, LOSSES, LIABILITIES, COSTS, EXPENSES, ATTORNEY'S FEES, JUDGMENTS, OR CLAIMS OF ANY KIND ARISING OUT OF OR RELATED TO MY PARTICIPATION OR MY CHILD(REN)'S PARTICIPATION IN ANY CORE AND CRADLE ACTIVITY.
THIS RELEASE INCLUDES CLAIMS ARISING FROM:
• Personal injury • Illness or disease • Death • Emotional distress • Property damage • Economic loss • Injuries involving children • Exercise and fitness activities • Food and beverage consumption • Third-party venues, vendors, sponsors, instructors, contractors, or service providers

TO THE FULLEST EXTENT PERMITTED BY LAW, THIS RELEASE APPLIES REGARDLESS OF WHETHER SUCH CLAIMS ARISE FROM NEGLIGENCE OR OTHERWISE.

5. THIRD-PARTY VENUES, VENDORS, SPONSORS, AND SERVICE PROVIDERS
I understand that Core and Cradle events may take place at third-party venues and may involve vendors, sponsors, exhibitors, photographers, videographers, food and beverage providers, soft play providers, wellness providers, entertainers, retailers, instructors, guest speakers, activity providers, and other independent service providers.

I acknowledge that Core and Cradle does not own, operate, supervise, inspect, control, or guarantee the conduct, products, services, equipment, facilities, food, beverages, or activities provided by such third parties.
I voluntarily assume all risks associated with participation in, use of, purchase of, or interaction with any third-party products, services, or activities.

6. GUEST INSTRUCTORS, INDEPENDENT CONTRACTORS, AND THIRD-PARTY FACILITATORS
I understand that Core and Cradle may engage guest instructors, fitness professionals, wellness practitioners, photographers, videographers, entertainers, educators, childcare activity providers, vendors, sponsors, speakers, exhibitors, and other independent contractors in connection with its classes, events, and activities.
I acknowledge that such individuals and entities may not be employees of Core and Cradle.
I agree that all releases, waivers, assumptions of risk, indemnification provisions, limitations of liability, and protections contained in this Agreement apply equally to and are intended to benefit such individuals and entities to the fullest extent permitted by law.
I voluntarily assume all risks associated with participating in activities led, facilitated, instructed, organized, sponsored, supported, or conducted by such individuals and entities.

7. FOOD, BEVERAGES, PRODUCTS, AND ALLERGIES
I understand that food, beverages, snacks, samples, promotional items, gifts, products, and sponsor-provided items may be available at Core and Cradle events.
Core and Cradle makes no representations regarding ingredients, allergens, preparation methods, cross-contamination, food safety, product safety, or suitability for use or consumption.
I assume full responsibility for any allergic reaction, illness, injury, adverse health event, or damages arising from the use or consumption of any food, beverage, product, sample, gift, or promotional item.

8. PROPERTY DAMAGE
I agree to be financially responsible for any damage caused by me or my child(ren) to venue property, event property, rental equipment, displays, furnishings, installations, equipment, or the property of other attendees.

9. EMERGENCY MEDICAL AUTHORIZATION
In the event of a medical emergency involving me or my child(ren), I authorize Core and Cradle to seek emergency medical treatment if deemed necessary.
I understand that Core and Cradle has no obligation to provide medical care and that I am solely responsible for any costs associated with medical treatment.

10. PHOTO, VIDEO, AND MEDIA RELEASE
I grant Core and Cradle the irrevocable and unrestricted right to photograph, film, record, and otherwise capture my image, likeness, voice, and the image, likeness, and voice of my child(ren).
I authorize Core and Cradle to use, reproduce, edit, publish, distribute, display, and otherwise use such content in any format or media, including websites, social media, advertising, marketing materials, promotional campaigns, public relations materials, and future event promotions without compensation, notice, or additional consent.

11. INDEMNIFICATION
I agree to indemnify, defend, and hold harmless Core and Cradle from and against any claims, liabilities, damages, losses, costs, expenses, or attorney's fees arising out of:
• My actions or omissions • My child(ren)'s actions or omissions • My violation of this Agreement • Claims brought on behalf of me or my child(ren)

12. GOVERNING LAW
This Agreement shall be governed by and interpreted in accordance with the laws of the State of Florida.

13. SEVERABILITY
If any provision of this Agreement is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

14. FUTURE PARTICIPATION
I understand and agree that this Agreement applies to my participation and my child(ren)'s participation in all current and future Core and Cradle classes, events, workshops, activations, and programming unless revoked in writing and acknowledged in writing by Core and Cradle.

15. ACKNOWLEDGMENT
I acknowledge that I have carefully read this Agreement, fully understand its contents, understand that I am waiving certain legal rights, and voluntarily agree to all terms contained herein.
