Join us for a fun, beginner-friendly Sunday social combining movement, music, and community. We'll kick things off with a signature M'Bodied Pilates session before learning the basics of pickleball with an instructor-led lesson and hitting the courts for open social play.

No experience? Perfect. This event is designed for beginners and anyone looking to try something new, meet new people, and have a good time.

PARTICIPANT WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK

Event Date: Sunday, July 19, 2026
Location: Gates of St. John Pickleball Courts, St. John, Indiana

Please read carefully before signing.

I, the undersigned participant, voluntarily choose to participate in DAY//PLAY: A Pilates + Pickleball Social ("Event"), hosted by M'Bodied Mvmt. I understand that the Event includes physical activities such as Pilates, pickleball instruction and play, stretching, recreational activities, food and beverage sampling, and participation in wellness activations and vendor experiences.

I understand that participation in these activities involves inherent risks, including but not limited to: slips, falls, collisions, muscle strains, sprains, fractures, dehydration, heat-related illness, allergic reactions, aggravation of pre-existing conditions, equipment-related injuries, property damage, and other serious bodily injury.

I knowingly and voluntarily assume all risks associated with my participation in the Event.

HEALTH ACKNOWLEDGMENT

I represent and warrant that:

• I am physically able to participate in the Event and have no medical condition that would prevent my safe participation, or I have consulted with my physician regarding my participation.

• I understand that it is my responsibility to monitor my physical condition and discontinue participation if I experience pain, dizziness, shortness of breath, or any concerning symptoms.

• I understand that participation in food and beverage sampling is voluntary and that I am solely responsible for identifying and avoiding ingredients that may cause allergies or sensitivities.

RELEASE OF LIABILITY

To the fullest extent permitted by law, I release, waive, discharge, and hold harmless M'Bodied Mvmt, Marisa Winslow, event staff, volunteers, instructors, DJs, vendors, sponsors, partners, the Town of St. John, and the owners, operators, employees, and representatives of the Gates of St. John Pickleball Courts from any and all claims, liabilities, demands, damages, losses, costs, expenses, or causes of action arising out of or related to my participation in the Event, including claims arising from ordinary negligence.

I understand and agree that I participate in this Event entirely at my own risk.

EMERGENCY MEDICAL AUTHORIZATION

I authorize Event staff to seek emergency medical treatment on my behalf if I become injured, ill, or unable to communicate during the Event. I understand that I am solely responsible for any costs associated with medical treatment or transportation.

PHOTO & VIDEO RELEASE

I understand that photographs, videos, and other recordings may be taken during the Event. I grant M'Bodied Mvmt and its representatives permission to use my image, likeness, voice, and appearance in photographs, videos, and promotional materials for marketing, advertising, social media, and future event promotion without compensation or further approval.

REFUND & EVENT POLICY

I understand that all ticket sales are final and non-refundable.

M'Bodied Mvmt reserves the right to modify, postpone, relocate, or cancel the Event due to inclement weather, safety concerns, or circumstances beyond its control. In such cases, ticket holders may be offered a credit, transfer, rescheduled date, or other remedy at the sole discretion of M'Bodied Mvmt.

PARTICIPANT ACKNOWLEDGMENTS

I understand that participation in this Event is voluntary and at my own risk.

I understand that participation in Pilates, pickleball, and all other activities is voluntary.

I understand that participation in food and beverage sampling is voluntary and that I am responsible for any allergies, dietary restrictions, or sensitivities.

I acknowledge that all ticket sales are final and non-refundable.

☐ I consent to being photographed and/or recorded for promotional purposes.

I certify that I am at least 18 years old or, if I am 16-17 years old, I understand that a signed parent or legal guardian waiver must be completed on the day of the Event before I may participate.

By signing below, I acknowledge that I have carefully read and fully understand this Waiver and Release of Liability and voluntarily agree to its terms.
