**Dinner on Purpose** - Come as strangers, leave as friends.

**What is it**: Take a short personality quiz and get matched into a group of 4-8 strangers whose personalities complement yours. You'll all meet for the first time at dinner. Restaurant location and limited info about your tablemates will be released a few days before dinner.

**READ CAREFULLY:** You will receive a link to the personality questionnaire in your email 1-2 days after payment. You MUST fill out the questionnaire by the deadline. Make sure your inbox is set to receive new messages (i.e. not full, and not an Apple private relay address). *If you've filled out the quiz in the past, you don't have to do it again.*

**Registration deadline is June 17 at 11:59pm.**

WAIVER AND RELEASE
By attending any event organized by Prime Time Social Club (“Prime Time”, “PTSC”, "our"), you agree to hereby waive and release, indemnify, hold harmless, and forever discharge PRIME TIME SOCIAL CLUB and its agents, officers, directors, affiliates, successors, and all of its constituents, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages, and liabilities, of every kind and nature, whether known or unknown, in law or equity, that you ever had or may have, arising from or in any way related to your participation in any of the events or activities, conducted, arranged, or hosted by PTSC.

By this Waiver, you assume any risk and take full responsibility and waive any claims of personal injury, death, or damage to personal property associated with engaging in Prime Time activities and events. 

The provision of this WAIVER AND RELEASE will continue in full force and effect even after the termination of the activities conducted by or for the benefit of PTSC, whether by agreement, by operation of law, or otherwise.

By participating in this event, you signify that you have read, understand, and fully agree to the terms of this WAIVER AND RELEASE. You agree that your participation in this event is voluntary, and you agree to this WAIVER AND RELEASE freely, voluntarily, under no duress or threat of duress, without inducement, promise, or guarantee being communicated to you. Your submission is proof of your intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law. You state that you are 18 years of age or older and mentally competent to enter into this waiver.

IMAGE RELEASE FORM
I hereby irrevocably consent to and authorize the use by Prime Time Social Club (“Prime Time”, “PTSC”) and all of its constituents, of any and all photographs, video, voice recordings, or other media taken of me including my likeness and derivative works thereof (collectively, the “Images”), and any reproduction of them in any form in any media whatsoever, whether now known or hereafter created, throughout the world in perpetuity, without payment or other consideration. I also consent to the use of my name or likeness, or an assigned fictitious name, in connection with the exhibition, distribution, merchandising, advertising, exploiting and/or publicizing of Images or PTSC.  I hereby release and discharge PTSC, its trustees, officers, employees, licensees, and affiliates from any and all claims, actions, suits or demands of any kind or nature whatsoever, in connection with the use of Images and the reproduction thereof as aforesaid. I understand and agree that PTSC will be the exclusive owner of all rights, including, but not limited to, all copyrights, in and to the Images in whole or part, throughout the universe, in perpetuity, in any medium now known or hereafter developed, and to license others to so use them in any manner PTSC may determine in its sole discretion, without any obligation to me. I hereby waive any right that I may have to inspect and/or approve the use of the Images or any reproductions thereof, by PTSC.
