**Hotel Haya** is a vibrant, independent 4-star lifestyle hotel in the heart of Ybor City. Our Discolates pop-up will take place inside the historic **Las Novedades** space—once the oldest restaurant in Tampa and later transformed into iconic nightclub concepts. Now reimagined as a multi-use venue, it’s full of charm, energy, and local history.

PLEASE READ CAREFULLY BEFORE SIGNING. BY SIGNING THIS DOCUMENT, YOU ACKNOWLEDGE AND AGREE TO THE FOLLOWING:

Assumption of Risk
I acknowledge that participation in physical fitness activities, including but not limited to Pilates and dance-based movement, carries inherent risks, including the risk of injury. I voluntarily assume all such risks associated with participating in the Discolates class, regardless of severity.

Medical Clearance
I affirm that I am physically fit and have no medical condition that would prevent my full participation in this activity. I will inform the instructor of any relevant health conditions or limitations prior to class.

Release of Liability
In consideration of being permitted to participate in this event, I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, waive, and hold harmless Bianca Falcon (Discolates), Hotel Haya, its owners, employees, affiliates, and agents from any and all liability, claims, demands, losses, or damages arising from or related to participation in this class or presence on the premises, whether caused by negligence or otherwise.

Use of Image and Likeness
I grant permission for my image, video, or likeness to be used in event promotion, social media, or marketing materials by Discolates and/or Hotel Haya. If I do not wish to be photographed or recorded, I will notify the instructor in advance.

Personal Belongings
I understand that I am responsible for my own personal items during the event and that Discolates and Hotel Haya are not responsible for lost, stolen, or damaged belongings.

Event Modifications and Cancellations
I acknowledge that Discolates and Hotel Haya reserve the right to make adjustments to the class format, location, or timing due to unforeseen circumstances, and that any changes will be communicated in a timely manner.

By signing below, I confirm that I have read this waiver and fully understand its contents. I agree to these terms voluntarily and acknowledge that this release of liability shall be binding to the fullest extent permitted by law.
