The Chiropractic golf Association and Player's Circle Golf Club Come together and support Semper Fi & America's Fund Nonprofit organization.

Semper Fi & America's Fund is a 501(c)3 nonprofit organization (Tax ID # 26-0086305) dedicated to providing assistance to our combat wounded, critically ill and injured service members, military

family members, and veterans, from all branches of the U.S. Armed Forces. Our case managers, visiting nurses, and entire team support those we serve by helping navigate

complex medical and emotional recovery — today, tomorrow, together!

In consideration of being allowed to participate in events organized by the Player Circle Golf Club, I hereby agree to the following terms and conditions:

Release of Liability:
I acknowledge that participation in golf events involves inherent risks, including the risk of injury. I agree that the Player Circle Golf Club, its members, and organizers shall not be held responsible for any injuries, losses, or damages that may occur during my participation.

Cheating Policy:
I understand that any act of cheating during events is strictly prohibited. If I am caught cheating, I acknowledge that I will no longer be invited to any future Player Circle Golf Club events.

Respectful Conduct:
I agree to represent the Player Circle Golf Club as a respectful member, treating all participants, organizers, and the game of golf with dignity and sportsmanship.

Photo & Media Release:
I grant permission to the Player Circle Golf Club to take photographs and videos of me during events. I understand that these images may be used for promotional, marketing, or other club-related purposes. I agree not to hold the Player Circle Golf Club, its members, or organizers liable for the use of these photos or videos, and I waive any right to compensation or legal action regarding their use.

By signing below, I confirm that I have read and understood this waiver and agreement, and I voluntarily agree to its terms.
