Mommy & Me Pilates – Waiver and Release of Liability
1. Acknowledgment of Risks
I understand that participating in any physical activity, including Pilates exercises, carries
inherent risks. I acknowledge that both I and my child may be engaging in activities involving
physical exertion that may result in injury. I confirm that I have consulted with a physician and
have no known medical conditions that would prevent me or my child from safely participating in
Pilates.
2. Voluntary Participation
I voluntarily agree to participate in the Mommy & Me Pilates class and allow my child to
participate as well. I understand that I am responsible for monitoring our own condition and
well-being throughout the session.
3. Release of Liability
I release and hold harmless [Anew Method LLC], its instructors, employees, and affiliates
from any and all claims, liabilities, damages, or expenses arising out of or in connection with
participation in the Pilates class. This includes any claims for personal injury, property damage,
or wrongful death, whether caused by negligence or otherwise.
4. Parental Consent and Responsibility
I understand I am responsible for my child’s behavior and safety during the class. I agree to
supervise and ensure my child is not endangering themselves or others. I understand this is not
a childcare service, and I must be present with my child at all times.
5. Photo/Video Release
I grant permission for [Anew Method LLC] to use photographs or videos of me and/or my
child for promotional purposes.
6. Agreement to Terms
By signing below, I confirm that I have read and fully understand this waiver and release. I
acknowledge that I am giving up legal rights by signing it, and I sign it freely and voluntaril
