FIT & FLOW FALL INTO WELLNESS — PARTICIPATION WAIVER & RELEASE

Event: Fit & Flow x Diamond Fitness — Fall Into Wellness

Event Organizer: Forteza Wellness / Fit & Flow Community
By registering for, attending, or participating in Fit & Flow x Diamond Fitness — Fall Into Wellness (“Event”), I acknowledge and agree to the following:

1. Assumption of Risk
I understand that participation in fitness classes, strength and conditioning activities, functional movement, stretching, mobility exercises, games, outdoor activities, use of fitness equipment, and other physical activities may involve inherent risks, including but not limited to muscle soreness, strains, sprains, falls, injuries, illness, property damage, or other physical injury.
I voluntarily choose to participate and understand that I am responsible for determining whether I am physically and medically able to participate safely.

2. Health & Medical Responsibility
I represent that I am physically capable of participating in the activities I choose to participate in. I agree to stop participating if I experience pain, dizziness, shortness of breath, weakness, or any other concerning symptom and, when appropriate, seek medical attention. I understand that the Event is not a substitute for medical advice, diagnosis, or treatment.

3. Release of Liability
To the fullest extent permitted by applicable law, I release and agree not to hold liable Forteza Wellness, Fit & Flow Community, Diamond Fitness, their respective owners, employees, coaches, instructors, contractors, volunteers, event staff, participating vendors, sponsors, and representatives (“Released Parties”) for claims arising from my voluntary participation in the Event, including claims relating to personal injury, illness, death, or property damage, except to the extent caused by conduct that cannot legally be released or waived.

4. Fitness Equipment & Facility Use
I agree to use all equipment and facilities responsibly and according to instructions provided by Diamond Fitness, Event staff, coaches, or instructors. I understand that improper or unsupervised use of equipment may result in injury.
I agree to follow all posted facility rules and reasonable instructions from Event staff.

5. Children & Minors
Participants under 18 must have a parent or legal guardian complete and accept this waiver on their behalf.
I understand that children and minors must remain under the supervision and responsibility of their parent or legal guardian throughout the Event unless otherwise specifically stated by Event staff.Parents/legal guardians are responsible for determining which activities are appropriate for their children.

6. Vendors, Sponsors & Third-Party Activities
The Event may include independent vendors, sponsors, fitness professionals, wellness providers, food/beverage providers, children's activities, games, and other third-party experiences.
I understand that these individuals or businesses may have their own rules, requirements, or waivers. My participation in any third-party activity may require an additional waiver or acknowledgment.

7. Emergency Medical Care
In the event of an emergency, I authorize Event staff to seek reasonable emergency medical assistance on my behalf when necessary. I understand that I am responsible for any medical, ambulance, hospital, or other healthcare expenses incurred on my behalf.

8. Personal Belongings
I understand that I am responsible for my personal belongings. The Released Parties are not responsible for lost, stolen, or damaged personal property except where liability cannot legally be excluded.

9. Photography & Video
I understand that photography and video recording may take place during the Event for promotional, social media, marketing, and educational purposes.

10. Event Rules & Conduct
I agree to follow reasonable Event, facility, safety, and conduct guidelines. I understand that the Event Organizer and/or venue may ask any participant to discontinue participation or leave the Event if their behavior creates a safety concern or disrupts the experience of others.

11. Acknowledgment & Agreement
By registering for the Event, I acknowledge that I have read and understand this waiver, that I am voluntarily participating in the Event, and that I accept the risks associated with participation.
I understand that this waiver applies to my participation in Fit & Flow x Diamond Fitness — Fall Into Wellness on October 24, 2026, including activities taking place indoors and outdoors.
