We’re giving back to: 
Phoenix Childrens Holiday Toy Drive (Please bring one new, unwrapped toy for entry)
Dog Rescue (100% of proceeds will be donated directly to the rescue) - You can bring dog supplies as well if you’d Like! 

This special event features of 45 minute holiday themed 45 minute holiday themed body sculpt + core workout, plus a full experience including:

By participating in this Flex N’ Squeeze event, I acknowledge and agree to the following:

1. ASSUMPTION OF RISK

I understand that participating in physical fitness activities, including but not limited to strength training, Pilates-based movements, cardio, stretching, and high-intensity exercises involves certain inherent risks. These risks may include, but are not limited to:
	•	Muscle soreness
	•	Sprains or strains
	•	Injuries due to improper form
	•	Falls or slips
	•	Dehydration
	•	Accidents occurring on the premises

I voluntarily choose to participate, and I accept full responsibility for my health and any risks of injury.

2. RELEASE OF LIABILITY

In consideration of being allowed to participate, I hereby release, waive, and discharge:

Flex N’ Squeeze LLC, Yara Solorzano, all event team members, volunteers, vendors, participating organizations, and the event venue

from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or health complications that may occur during or after the event.

I confirm that I am participating at my own risk.

3. FITNESS & HEALTH CERTIFICATION

I certify that:
	•	I am physically capable of participating in this event.
	•	I have no medical conditions that would prevent safe participation.
	•	I will stop immediately if I feel pain, dizziness, or discomfort.
	•	I am solely responsible for monitoring my physical condition during the event.

If I have any health concerns, I agree to consult a physician before participating.

4. PERSONAL PROPERTY

I understand that Flex N’ Squeeze LLC and the venue are not responsible for:
	•	Lost, stolen, or damaged personal items
	•	Property left behind or unattended

5. PHOTO, VIDEO & MEDIA RELEASE

I grant permission for Flex N’ Squeeze LLC, Yara Solorzano, vendors, and event photographers to capture and use photos and videos of me from the event for:
	•	Social media
	•	Marketing
	•	Advertising
	•	Website content
	•	Press or promotional materials

I understand no compensation is provided for use of this content.

6. CHARITY DISCLOSURE

I acknowledge that this event supports:
	•	Phoenix Children’s Holiday Toy Drive (donation of one new, unwrapped toy required for entry)
	•	Dog Rescue (all event proceeds will be donated)

I agree that all donations and proceeds are final and non-refundable.

7. COVID-19 / GENERAL ILLNESS RELEASE

I certify that I will not attend if I am experiencing symptoms of illness.
I release Flex N’ Squeeze LLC from any claims related to viral or infectious disease exposure.
