I affirm that I alone am responsible to decide whether I am able to participate. I hereby agree, for both myself and other person(s) or child(ren) I am responsible for, to release and waive any claims that I have now or hereafter may have against the Group/Instructor, Sound Off™, and Venue and its affiliates participating in this event.

I grant the Group/Instructor and Sound Off™, in perpetuity, the irrevocable right (but not the obligation), with or without my knowledge, to film, tape, photograph, record, exhibit, edit, alter, copy, reproduce, license, sell, rent, disclose, display, publish, distribute, broadcast, webcast, prepare derivative works from or otherwise preserve, use and/or exploit in any format and/or manner now known or hereafter developed, whether commercial or non-commercial in nature (collectively, the “Use and Materials”): (1) my appearance at and/or participation in a Group/Instructor or Sound Off™ event, (2) my name, likeness, biographical data and/or any other information or material secured by Group/Instructor or Sound Off™.
