Assumption of Risk

I understand that participating in Flow With Pita classes involves physical activity, including yoga-inspired movement, strength training, stretching, balance exercises, and the use of light weights.

I understand that these activities carry inherent risks, including but not limited to muscle soreness, strains, sprains, falls, fractures, cardiovascular events, or other injuries.

I voluntarily choose to participate and assume all risks associated with my participation.

Health Responsibility

I certify that I am physically able to participate.

I understand that it is my responsibility to:

* exercise within my own limits
* stop immediately if I experience pain, dizziness, shortness of breath, or discomfort
* consult my physician if I have any medical concerns before participating

If I am pregnant, recently postpartum, recovering from an injury, or have any medical condition, I understand that participation is entirely my responsibility.

Release of Liability

In consideration for being allowed to participate, I release and hold harmless:

its owner, instructors, assistants, volunteers, and the hosting venue

from any and all liability, claims, demands, damages, costs, or causes of action arising out of my participation, except where prohibited by law or resulting from gross negligence or intentional misconduct.

Photo & Video Release

I understand that photos and videos may be taken during class for marketing, promotional, educational, and social media purposes.

By participating, I grant Flow With Pita permission to use my image, likeness, voice, and video recordings without compensation.

If I prefer not to appear in photos or videos, I will notify the instructor before class begins, and reasonable efforts will be made to accommodate my request.
