Please bring your own mat and water.&#x20;

In consideration of being allowed to participate in the fitness class hosted by Plu Ultra Corp, FS8, and F45 Arizona Premier (collectively, the “Organizers”), I hereby acknowledge and agree to the following:

Assumption of Risk
I understand that participation in fitness activities, including but not limited to Pilates and related exercises, involves inherent risks, including risk of injury, illness, or other adverse health events.
I voluntarily assume all risks, known and unknown, associated with participation in the Event.

Health Certification
I certify that I am physically fit to participate and have not been advised otherwise by a qualified medical professional.
I understand it is my responsibility to consult with a physician regarding my fitness to participate.

Release of Liability
I release and discharge Plu Ultra Corp, FS8, F45 Arizona Premier, their respective owners, officers, employees, contractors, event organizers, sponsors, affiliates, and volunteers from any and all liability, claims, demands, actions, or causes of action related to any loss, damage, injury, or illness that may occur during or as a result of my participation.

This release extends to claims arising from negligence, but does not waive claims of gross negligence or willful misconduct under applicable law.

Indemnification
I agree to indemnify, defend, and hold harmless Plu Ultra Corp, FS8, F45 Arizona Premier, and their affiliates, from any claims brought by third parties arising from my actions during participation in the Event.

Photography/Media Release
I grant permission for photos, videos, and other media taken during the Event that include my image or likeness to be used by Plu Ultra Corp, FS8, and F45 Arizona Premier for promotional purposes, without compensation.

Acknowledgment of Public Setting
I acknowledge that this Event takes place outdoors at Tempe Town Lake, which is a public space. I am responsible for my own personal property, hydration, and equipment (including mat and water).

Binding Effect
This agreement is binding on me, my heirs, executors, administrators, and assigns.
If any portion of this waiver is found unenforceable, the remaining provisions shall remain in full force and effect.

I acknowledge that I have read and understood this Waiver and Release of Liability, and I voluntarily agree to its terms.
