Because this session is for intermediate and advanced players we strongly discourage beginners from joining!

1. Assumption of Risk: I acknowledge that participation in volleyball involves inherent risks, including but not limited to bodily injury, illness, permanent disability, paralysis, or death. I knowingly and freely assume all such risks, both known and unknown, associated with my participation.

Release and Discharge: I, for myself, my heirs, personal representatives, or assigns, hereby release, discharge, and hold harmless SERVED, its owners, directors, officers, employees, and volunteers from any and all liability, claims, or demands arising out of negligence, fault, or misconduct on the part of the released parties.

3. Medical Treatment: I consent to receive medical treatment that may be deemed advisable in the event of injury, accident, or illness during this activity.

4. Participant Signature: I certify that I am at least 18 years of age and legally competent to sign this agreement.
