This beautifully designed journal makes mindfulness effortless with guided prompts, 200 lifestyle tips, and sections for gratitude, clarity, and reflection. It’s the perfect companion to help you stay grounded and inspired throughout the year.

RELEASE OF LIABILITY WAIVER
By signing this legal document, you are waiving any legal rights you may have to sue The Unplugged Collective LLC, its owners, representatives, employees, contractors, sponsors, or any associated parties in court for money or other damages.

ASSUMPTION OF THE RISKS
I, the undersigned participant, acknowledge and understand that participation in the event hosted by The Unplugged Collective LLC, including but not limited to yoga, journaling, food and beverage consumption, and social activities (collectively, "Activities"), involves inherent risks. These risks may include, but are not limited to, physical injury, illness, allergic reactions, and other unforeseen hazards.

PARTICIPATION AND MEDICAL RESPONSIBILITY
I affirm that:
I possess the necessary skills and physical fitness to safely participate in the Activities. I have no known medical conditions that would prevent me from safely participating. I will not participate in any Activities under the influence of alcohol, drugs, or any substance that may impair my ability to participate safely.
I understand the potential risks associated with food and beverage consumption, including allergic reactions and foodborne illnesses. I will inform The Unplugged Collective LLC of any known allergies or medical conditions that may affect my participation.

I agree to:
Exercise reasonable care during participation. Follow all instructions and safety guidelines provided by The Unplugged Collective LLC and its representatives. Cease participation immediately if I experience any unusual discomfort or symptoms.
Assume all costs of emergency medical care and transportation, if necessary.

PHOTOGRAPH LICENSE
I irrevocably grant The Unplugged Collective LLC the right to use images or videos of me, as well as my name and likeness, in all forms and media for advertising, trade, or any commercial purpose. I waive the right to inspect or approve any version of images or videos used for publication. I release The Unplugged Collective LLC from any claims that may arise regarding the use of my statements, videos, or images, including claims of defamation, invasion of privacy, or infringement of rights.

AGREEMENT TO FOLLOW DIRECTIONS
I agree to obey all rules and directions given by The Unplugged Collective LLC and its representatives while participating in the Activities. Failure to follow these rules may result in removal from the event without a refund.

RELEASE
In consideration of being allowed to participate in the Activities, I, on behalf of myself, my heirs, executors, and assigns, hereby release, waive, discharge, and covenant not to sue The Unplugged Collective LLC, its owners, representatives, employees, contractors, sponsors, or any associated parties from any and all liabilities, claims, or actions arising from any loss, damage, or injury, including death, that may be sustained by me or my property while participating in the Activities.

INDEMNIFICATION
I agree to indemnify, defend, and hold harmless The Unplugged Collective LLC from any claims, liabilities, damages, or expenses arising out of or related to my participation in the Activities.

COVID-19 SPECIFICS
I acknowledge the risks associated with COVID-19 and agree to comply with all health and safety guidelines implemented by The Unplugged Collective LLC. I assume all risks of potential exposure during the event.

EMERGENCY PROCEDURES
In the event of an emergency, I agree to follow all instructions of The Unplugged Collective LLC staff. I acknowledge that emergency procedures may include summoning medical services and providing first aid as necessary.

SEVERABILITY CLAUSE
If any part of this Waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

JURISDICTION AND VENUE
This Waiver shall be governed by the laws of the state of Florida. Any disputes arising under this Waiver shall be resolved in the appropriate courts of Florida.

CONFIRMATION AND ACKNOWLEDGMENT
By signing below, I acknowledge that I have read this Waiver, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I sign this document freely and voluntarily, intending it to be a complete and unconditional release of all liability to the greatest extent allowed by law.
