Glutes & Cold Brew is a Sunday morning glute workout + cold brew social for women, proudly sponsored by **1st Phorm**! This is a fun, beginner-friendly glute workout followed by a cold brew social where you can meet new people, hang with the community, and enjoy your Sunday morning with the girls.

This monthly event is open to **members and non-members**, so whether you're a regular at the gym or just looking for a fun, active Sunday morning, you're welcome here!



## THE WORKOUT

Expect a glute & lower-body focused workout designed to challenge you without feeling intimidating. We'll work through a variety of strength-based movements in a fun, encouraging environment.

**No experience required — all fitness levels are welcome!**

After the workout, stick around for cold brew and community!

We're proud to partner with **1st Phorm**, who sponsors Glutes & Cold Brew. Our cold brews are made with **1st Phorm protein**, giving you a delicious, protein-packed post-workout drink to enjoy while you hang out with the crew.

Liability Waiver & Assumption of Risk Agreement
1. Acknowledgment of Risk

I understand that participation in fitness activities at 3 Form Fitness, including but not limited to:

Strength training
Group fitness classes
Personal training
Injury Prevention & Rehab sessions
Mobility work
Cardiovascular training
Use of gym equipment
Recovery modalities (including compression devices, manual therapy tools, and other recovery services)

involves inherent risks.

These risks may include, but are not limited to:

Muscle strains or tears
Ligament sprains
Joint injuries
Falls
Equipment-related injuries
Dizziness or fainting
Cardiovascular events
Serious bodily injury
Permanent disability
Death

I voluntarily choose to participate with full knowledge and understanding of these risks.


2. Assumption of Risk

I knowingly and voluntarily assume full responsibility for all risks associated with participation in activities at 3 Form Fitness.

I understand that even with proper instruction, supervision, equipment, and safety precautions, injuries may still occur.


3. Medical Acknowledgment

I affirm that:

I am physically able to participate in fitness activities.
I have disclosed any known medical conditions that may affect my ability to safely participate.
I understand it is my responsibility to consult with a physician prior to beginning any exercise program if I have concerns.

I agree to immediately stop any activity if I experience pain, dizziness, shortness of breath, or other concerning symptoms.


4. Release of Liability

In consideration of being allowed to participate in activities at 3 Form Fitness, I hereby release, waive, and discharge:

3 Form Fitness- Sarasota, LLC
RCW Enterprises, LLC
Its owners, employees, contractors, trainers, and affiliates

from any and all liability, claims, demands, causes of action, or damages arising out of or related to any injury, loss, or damage sustained while participating in activities at the facility or using its equipment.

This release applies to any claims arising from negligence, except in cases of gross negligence or intentional misconduct.


5. Indemnification

I agree to indemnify and hold harmless 3 Form Fitness- Sarasota, LLC and its representatives from any claims, damages, or legal expenses arising from my participation.


6. Photo & Media Release (Optional Section – Can Separate If Preferred)

I grant permission for 3 Form Fitness- Sarasota, LLC to use photographs or videos taken during training sessions for marketing or promotional purposes, without compensation.


7. Emergency Medical Treatment

I authorize 3 Form Fitness staff to seek emergency medical treatment on my behalf if necessary. I understand that I am responsible for any associated medical costs.


8. Governing Law

This agreement shall be governed by the laws of the State of Florida.

Any disputes shall be resolved in Sarasota County, Florida.


9. Severability

If any portion of this agreement is found to be unenforceable, the remaining portions shall remain in full force and effect.


10. Acknowledgment of Understanding

I have carefully read this Liability Waiver and Assumption of Risk Agreement.

I understand its contents and sign it voluntarily.
