1. ACKNOWLEDGMENT OF RISK
I understand that this event ("Event"), hosted by Halo House, is an outdoor gathering that may include, without limitation: an open bar and consumption of alcoholic beverages, food service, a swimming pool and pool area, a tattoo artist offering tattoo and/or body art services, a Pilates mat class or other physical exercise activity, live music or entertainment, and general mingling on the premises. I understand that participation in any or all of these activities carries inherent risks, including but not limited to: slipping or falling (including on wet or pool-adjacent surfaces), drowning or near-drowning, injury from physical exercise, allergic or adverse reactions to food or beverages, impairment or injury related to alcohol consumption, infection, scarring, allergic reaction, or other complications arising from tattoo or body art procedures, and general risks associated with attending a group social event.

I acknowledge that these risks cannot be eliminated regardless of the care taken by Halo House, its owners, employees, contractors, vendors, and agents (collectively, the "Released Parties") to avoid injury.

2. ALCOHOL ACKNOWLEDGMENT
I confirm that if I choose to consume alcoholic beverages at this Event, I am 21 years of age or older. I understand that Halo House and its bartenders/servers reserve the right to refuse service to any individual. I am solely responsible for my own alcohol consumption and its effects, and for arranging safe transportation from the Event. I agree not to drive or operate a vehicle if I am impaired.

3. TATTOO / BODY ART SERVICES
I understand that any tattoo artist present at the Event is an independent contractor/vendor and not an employee or agent of Halo House. I understand that any tattoo or body art service is a separate transaction between me and that vendor, that I will be asked to complete that vendor's own consent and release form before receiving any service, and that Halo House is not responsible for the performance, outcome, sanitation practices, or any injury, infection, or dissatisfaction arising from services performed by that vendor.

4. SWIMMING POOL
I understand that a swimming pool will be present at the Event and that swimming or entering the pool area is done entirely at my own risk. Unless otherwise posted on-site, I understand there may be no lifeguard on duty. I agree to supervise myself (and any guest or minor in my care) around the pool at all times, and I agree not to enter the pool while impaired.

5. RELEASE OF LIABILITY AND INDEMNIFICATION
In consideration of being permitted to attend and participate in the Event, I, on behalf of myself and my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, illness, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in the Event, or while on the premises where the Event is being held, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

I further agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur arising out of my participation in the Event, whether caused by my own negligence or the negligence of others.

Nothing in this waiver is intended to, nor shall it, release any party from liability for gross negligence, willful misconduct, or any liability that cannot be waived as a matter of law.

6. PERSONAL PROPERTY AND PARKING
Halo House is not responsible for any loss, theft, or damage to personal property, vehicles, or belongings brought to or left at the Event, including in any parking area, whether during or after the Event. Parking, where available, is at my own risk.

7. NO LIABILITY AFTER THE EVENT
I understand and agree that Halo House's responsibility for the Event is limited to the scheduled duration of the Event itself, and that Halo House assumes no liability for any incident, injury, loss, or claim arising after the Event has concluded or after I have left the premises.

8. MEDICAL TREATMENT AUTHORIZATION
In the event of injury or illness, I authorize Halo House staff to seek emergency medical treatment on my behalf if I am unable to do so myself, and I agree that I am responsible for any costs associated with such treatment.

9. PHOTO/VIDEO RELEASE
I grant Halo House permission to photograph and/or video record me at the Event and to use those images for promotional or marketing purposes, unless I notify Halo House staff in writing prior to the Event that I decline this permission.

10. GENERAL
I have read this waiver, fully understand its terms, and sign it freely and voluntarily. This waiver is governed by the laws of the State of Florida. If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.

I confirm I am 18 years of age or older.
