We're doing an hour of heated sculpt built to the beat, then we celebrate at the complimentary coffee & matcha bar. No tab required!



The class takes you through a core series, upper and lower body strength, and builds into full-body flows.



**Here's what to know:** grip socks and towels are required, mats & equipment are provided, all sales are final & non-refundable — if plans change, just let me know 🫶

AMANDA x DUO SCULPT LIABILITY WAIVER & RELEASE

By signing up for and/or participating in the Amanda x Duo Sculpt class (“Class”) located at 16411 Bellflower Blvd, Bellflower, CA, I acknowledge and agree to the following:

1. Assumption of Risk
I understand that participation in fitness classes, including Heated Sculpt, involves physical activity in a heated environment and carries inherent risks, including but not limited to muscle injury, dehydration, dizziness, fainting, or more serious injury. I voluntarily assume all risks associated with my participation.

2. Health & Physical Condition
I certify that I am physically able to participate in this Class and have no medical condition that would prevent my safe participation. I understand it is my responsibility to consult with a physician prior to participating if I have any concerns.

3. Release of Liability
I hereby release, waive, and discharge Amanda Martinez, all instructors, and the hosting studio at 16411 Bellflower Blvd, Bellflower, CA from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or damages that may occur as a result of my participation in the Class.

4. Personal Responsibility
I agree to listen to my body, modify exercises as needed, take breaks when necessary, and stay hydrated. I understand that I am responsible for my own well-being during the Class.

5. No Guarantees
I understand that results from participating in the Class are not guaranteed.

6. Emergency Medical Treatment
I authorize the instructor or staff to obtain emergency medical treatment for me if necessary and agree to be responsible for any associated costs.

7. Photo/Video Release
I grant permission for photos or videos taken during the Class to be used for promotional purposes. 

By purchasing a ticket and/or attending the Class, I acknowledge that I have read, understood, and agree to this waiver.
