LIABILITY WAIVER AND RELEASE FORM
Las Olas Wellness Weekend

Date of Event: September 13, September 14
Location: Downtown Ft. Lauderdale, 221 SW 1st. Ave. 

In consideration of being allowed to participate in any way in the Las Olas Wellness Weekend, including but not limited to fitness activities, use of the dog park, and all other event-related activities, the undersigned acknowledges, appreciates, and agrees to the following on behalf of themselves and any minors or pets they bring:

Assumption of Risk: I acknowledge that participation in physical activities, including but not limited to exercise, fitness classes, and activities involving pets in a dog park, involves inherent risks, including the risk of serious injury, illness, permanent disability, or death. I knowingly and voluntarily assume all such risks, both known and unknown, even if arising from the negligence of the Released Parties or others.

Release and Waiver: I hereby release, waive, discharge, and hold harmless the following organizations and their respective directors, officers, employees, volunteers, representatives, agents, affiliates, successors, and assigns (collectively, the “Released Parties”):

from any and all claims, liabilities, demands, damages, losses, or causes of action whatsoever arising out of or related to any injury, disability, death, or loss or damage to person or property that may be sustained by me (or my child or pet) while participating in or attending the event.

Medical Treatment: I understand that the Released Parties do not provide medical services. I consent to receive medical treatment which may be deemed advisable in the event of injury, accident, or illness during the event. I agree to be solely responsible for all medical expenses incurred.

Dog Park and Animal Safety: If I bring a pet to the event, I certify that it is in good health, up to date on vaccinations, non-aggressive, and under my control at all times. I accept full responsibility for my pet’s behavior and any injury or damage it causes to persons, property, or other animals.

Photography & Media Release: I understand that photos and videos may be taken at the event and used for promotional purposes. I grant permission to the Released Parties to use my likeness without compensation.

Legal Capacity: I affirm that I am at least 18 years old and legally competent to sign this agreement. If I am signing on behalf of a minor, I certify that I am the minor's parent or legal guardian and have the legal capacity to sign this release on their behalf.

BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ THIS RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY.
