I understand that participation in fitness classes, functional training, and related physical activities involves inherent risks. These risks may include, but are not limited to: slips, falls, sprains, strains, muscle injuries, dehydration, equipment use, and other potential physical harm. I voluntarily choose to participate, acknowledging these risks.

Release of Liability

In consideration of being allowed to participate in events, programs, and activities hosted by Vault Fitness, I, for myself, my heirs, executors, and administrators, hereby release and hold harmless Vault Fitness, its owners, instructors, staff, volunteers, and affiliates from any and all liability, claims, demands, or actions that may arise from my participation. This includes claims related to injury, illness, property damage, or death, whether caused by negligence or otherwise.

Medical Clearance

I affirm that I am physically fit to participate in these activities and have no medical condition that would prevent my safe participation. I understand that it is my responsibility to consult with a physician prior to participation if I have any health concerns. In the event of a medical emergency, I authorize event personnel to secure medical treatment for me, and I assume responsibility for any related costs.

I grant Vault Fitness the right to photograph, film, or record me during events and to use such media for promotional and marketing purposes without compensation.
