LIABILITY WAIVER AND RELEASE AGREEMENT
Outdoor Pilates Event
This Liability Waiver and Release Agreement (“Agreement”) is entered into by the undersigned
Participant and/or Legal Guardian (“Participant”) in favor of the event organizer, instructors,
employees, volunteers, agents, and independent contractors (“Releasees”). By signing below,
Participant acknowledges and agrees to the following:
Section I: Assumption of Risk (Physical Activity & Outdoor
Environment)
Participant acknowledges that participation in Pilates classes, workshops, or events involves
strenuous physical activity that may result in injury or illness, including but not limited to: muscle
strains, sprains, broken bones, heat exhaustion, dehydration, slips and falls, or other physical
injuries. Because this is an outdoor event, Participant understands and voluntarily assumes
additional risks including uneven ground, weather conditions, insects, allergens, and outdoor
equipment or surface-related hazards. Participant voluntarily assumes all such risks, whether
known or unknown.
Section II: Health and Medical Acknowledgment
Participant affirms that they (and/or their minor children) are in good physical condition and able to
participate in physical activity. Participant agrees to consult a medical professional regarding any
concerns prior to participation and will inform the event organizer of any medical conditions,
injuries, allergies, or limitations that may affect safe participation. Participation is voluntary and at
Participant’s own risk.
Section III: Hands-On Assistance Consent
Participant understands that instructors may offer hands-on assistance or physical adjustments
during instruction. Participant consents to such professional contact and understands that they may
decline adjustments at any time.
Section IV: Food & Beverage Acknowledgment
Participant acknowledges that drinks and snacks may be provided during the event. Participant
understands they are responsible for monitoring any food allergies, sensitivities, or dietary
restrictions. The event organizer is not responsible for allergic reactions, illness, or adverse effects
related to food or beverage consumption. Consumption of any provided items is voluntary and at
Participant’s own risk.
Section V: Release of Liability
Participant, on behalf of themselves, their heirs, representatives, and assigns, hereby fully releases
and forever discharges the event organizer and all Releasees from any and all claims, demands,
damages, causes of action, or liability for any injury, illness, loss, or damage to person or property
arising from participation in any activity or presence at the event, including those caused by
negligence to the fullest extent permitted by Florida law.
Section VI: Indemnification
Participant agrees to indemnify and hold harmless the event organizer from any loss, liability,
damage, or costs incurred due to Participant’s participation, conduct, or presence at the event,
including claims brought by third parties related to Participant’s actions.
Section VII: Minor Participants
If signing on behalf of a minor child, Participant certifies they are the legal guardian and agrees to
all terms on behalf of the minor. Guardian assumes full responsibility for the minor’s participation
and safety.
Section VIII: Photo & Video Release
Participant grants permission for photographs and/or videos taken during the event to be used by
the event organizer for marketing, advertising, social media, or promotional purposes without
compensation. Participant understands this authorization may be revoked at any time in writing;
however, prior uses will remain valid.
Section IX: Severability & Governing Law
This Agreement is intended to be as broad and inclusive as permitted by the laws of the State of
Florida. If any portion is found invalid, the remaining provisions shall remain in full force and effect.
This Agreement shall be governed by Florida law.
Section X: Acknowledgment and Signature
Participant acknowledges that they have read, understood, and voluntarily agree to this Liability
Waiver and Release Agreement.
