Expect a full-body Pilates-inspired sculpt session paired with our signature lower-body strength and glute-focused training. All set to the hip-hop and R\&B beats you know and love!

# M’BODIED MVMT
**PARTICIPANT WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK**

**Event:** M’Bodied Community Pop-Up: Sculpt × Peach, Please!

**Date:** Sunday, October 18, 2026

**Location:** Dean & Barbara White Community Center, 6600 Broadway, Merrillville, IN 46410

### 1. Acknowledgment and Assumption of Risk

I voluntarily choose to participate in fitness activities provided by M’Bodied Mvmt, including but not limited to Pilates-inspired movement, strength training, resistance exercises, cardiovascular conditioning, and other physical activities.

I understand that participation involves inherent risks, including muscle soreness, strains, sprains, falls, dizziness, cardiovascular complications, and other injuries, potentially including serious injury or death.

I knowingly and voluntarily assume the inherent risks associated with participation.

### 2. Health and Physical Readiness

I acknowledge that I am responsible for determining whether I am physically capable of participating.

I agree to inform the instructor of any relevant injuries, medical conditions, physical limitations, or other concerns that may affect my ability to exercise safely.

I understand that I may modify, pause, or discontinue any exercise at any time.

I understand that M’Bodied Mvmt does not provide medical advice and that I should consult a qualified healthcare professional if I have concerns about participating.

### 3. Release of Liability

To the fullest extent permitted by Indiana law, I release and hold harmless M’Bodied Mvmt, its owner, instructors, employees, contractors, and representatives from claims arising from the ordinary negligence of the released parties in connection with my participation in the event.

This release does not apply to gross negligence, reckless conduct, intentional misconduct, or any liability that cannot legally be waived.

### 4. Emergency Medical Authorization

In the event of an injury or medical emergency, I authorize M’Bodied Mvmt representatives to contact emergency medical services if reasonably necessary.

I understand that I am responsible for any medical expenses incurred as a result of my participation.

### 5. Personal Property

I understand that I am responsible for my personal belongings and that M’Bodied Mvmt is not responsible for loss, theft, or damage except where liability cannot legally be excluded.

### 6. Photography and Video Consent

I understand that photographs and videos may be captured during the event for M’Bodied Mvmt marketing, social media, and promotional purposes.

I may separately consent to the use of my identifiable image and likeness for these purposes without compensation. Declining consent will not affect my ability to participate.

### 7. Participant Agreement

By signing or electronically accepting this waiver, I acknowledge that:

- I have read and understand this agreement.
- I voluntarily choose to participate.
- I understand the physical risks involved.
- I understand that I am giving up certain legal rights by agreeing to the release of liability.
- I agree to follow reasonable safety instructions provided by the instructor.
