M’Bodied Mvmt LLC – Participant Waiver & Release of Liability

By signing below, I acknowledge that I am voluntarily participating in a fitness event hosted by M’Bodied Mvmt LLC, which may include but is not limited to dance fitness, strength training, Pilates-inspired movement, and other physical activities.

1. ASSUMPTION OF RISK

I understand that participation in physical activity involves inherent risks, including but not limited to injury, muscle strain, falls, or other physical harm. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.

2. MEDICAL ACKNOWLEDGMENT

I confirm that I am physically able to participate in this activity and have no medical conditions that would prevent safe participation. I understand it is my responsibility to consult a physician if I have any concerns.

3. RELEASE OF LIABILITY

I hereby release, waive, and discharge M’Bodied Mvmt LLC, its owner(s), instructors, partners, collaborators (including but not limited to Hot Body Pilates and event venues), and any affiliated staff from any and all liability, claims, demands, or causes of action arising out of or related to any injury, damage, or loss that may occur during or after participation.

4. PHOTO & VIDEO RELEASE

I understand that photos and videos may be taken during the event for promotional and marketing purposes. By signing this waiver, I grant permission to M’Bodied Mvmt LLC to use my image, likeness, and/or voice in content without compensation.

5. CODE OF CONDUCT

I agree to conduct myself in a respectful and appropriate manner during the event. M’Bodied Mvmt LLC reserves the right to remove any participant for inappropriate behavior without refund.

6. NO REFUND POLICY

I understand that all ticket purchases are final and non-refundable unless otherwise stated.

7. ACKNOWLEDGMENT & AGREEMENT

By signing below, I acknowledge that I have read, understood, and agree to all terms of this waiver and release of liability.
