PARTICIPATION WAIVER, RELEASE OF LIABILITY, AND MEDIA CONSENT

By registering for, attending, or participating in Mindset & Movement, you acknowledge and agree to the following:

Acknowledgment of Participation

Mindset & Movement is a community-based program that may include:

• Life coaching discussions
• Personal development activities
• Goal setting and mindset coaching
• Group conversations
• Wellness education
• Walking
• Stretching
• Yoga
• Pilates
• Dance
• Strength training
• Water aerobics
• Swimming
• Breathwork
• Mindfulness activities
• Recreational fitness activities
• Outdoor wellness activities
• Other physical and wellness-related activities

Activities may vary from session to session and participation is always voluntary.

Assumption of Risk

I understand that participation in coaching, wellness, fitness, and community activities involves inherent risks.

These risks may include, but are not limited to:

• Slips, trips, and falls
• Muscle strains and sprains
• Cuts and abrasions
• Physical injury
• Allergic reactions
• Heat exhaustion
• Heat stroke
• Dehydration
• Water-related accidents
• Drowning
• Emotional distress
• Property damage
• Permanent disability
• Death

I voluntarily assume all risks associated with participation.

Medical Acknowledgment

I certify that I am physically and mentally capable of participating.

I understand that I am responsible for determining whether participation is appropriate for me.

I understand that organizers, coaches, instructors, and volunteers are not medical professionals and do not provide medical advice, diagnosis, treatment, therapy, counseling, or healthcare services.

If I have concerns regarding my physical or mental health, I agree to consult an appropriate healthcare professional before participating.

Life Coaching Disclaimer

I understand that Her Space Life Coaching LLC provides coaching services only.

Coaching is not:

• Therapy
• Counseling
• Psychology
• Psychiatry
• Mental health treatment
• Medical treatment
• Crisis intervention

All discussions, coaching sessions, educational content, and group conversations are intended solely for educational, informational, and personal development purposes.

I understand that no guarantees are made regarding personal growth, business outcomes, relationships, finances, health, or life results.

Fitness Participation

I understand that movement activities are led by instructors and may change from week to week.

Activities may include yoga, Pilates, stretching, walking, water aerobics, fitness classes, mobility work, recreational activities, and other wellness-related exercises.

I agree to participate only within my own abilities and limitations and may stop participation at any time.

Release of Liability

To the fullest extent permitted by law, I release, waive, and hold harmless:

Her Space Life Coaching LLC, 12 Circle Fitness, The Local Habit Community Initiative Inc., their owners, officers, directors, employees, contractors, coaches, instructors, volunteers, representatives, sponsors, venue hosts, partners, and affiliates

from any and all claims, liabilities, damages, losses, costs, expenses, injuries, illnesses, disabilities, or death arising from or related to my participation in Mindset & Movement.

This release includes claims arising from:

• Physical activities
• Coaching discussions
• Emotional wellness activities
• Venue conditions
• Outdoor activities
• Water activities
• Transportation
• Equipment usage
• Acts of other participants
• Acts of third parties
• Ordinary negligence

I understand that I am participating entirely at my own risk.

Personal Property

I understand that organizers are not responsible for lost, stolen, damaged, or misplaced personal property, including vehicles and personal belongings.

Photography & Media Release

By attending this event, I grant permission to Her Space Life Coaching LLC, 12 Circle Fitness, and The Local Habit Community Initiative Inc. to photograph, video record, and otherwise capture my image, voice, likeness, and participation.

I authorize these materials to be used for:

• Social media
• Marketing
• Advertising
• Websites
• Newsletters
• Promotional materials
• Public relations
• Fundraising efforts

I understand that I will not receive compensation for such use.

Code of Conduct

All participants agree to:

• Treat others with respect
• Maintain a welcoming environment
• Respect confidentiality when personal stories are shared
• Follow instructions from organizers and instructors
• Refrain from harassment, discrimination, bullying, or disruptive behavior

Organizers reserve the right to remove any participant whose behavior is deemed unsafe, disruptive, threatening, or inappropriate.

Agreement

By registering for, purchasing a ticket to, or attending Mindset & Movement, I acknowledge that:

✓ I have read and understood this waiver.

✓ I voluntarily assume all risks associated with participation.

✓ I release the organizations and individuals listed above from liability to the fullest extent permitted by law.

✓ I understand that coaching services are not therapy or medical treatment.

✓ I agree to all terms stated above.
